520			_aAnterior cruciate ligament injury is one of the most common knee injuries specially in athletes. Many methods were invented for ACL reconstruction , these methods varied in types of graft (synthetic and natural) , autografts or allograft , the autografts gave better results than synthetic or allograft, and between different types of autografts the hamstring tendons gave best results with minimal graft site morbidity . Many methods of fixation were used (cortical screws, threads, washers, interference screws, mini plates,) The interference screw method considered as the gold stander methods of fixation that also used to compare it and the newly invented methods, but this interference screw had its own problems like, screw breakdown, it is a foreign body, bone tunnel dilatation, it interfere with MRI and CT if it is needed postoperatively, also the revision ACL surgery became difficult with screws in addition to the financial side. A biological method of fixation were developed to fix the ACL graft using a bone plug that fix the graft by press fit technique , it has many advantages like it is a biological method no foreign body , decrease bone tunnel dilatation , it didn’t interfere with MRI or CT postoperatively , easy revision surgery , in addition to low coast. In this study we compare between the press fit technique and interference screw technique from clinical point of view to determine whether this method is effective and valuable or not. This a comparative prospective study done on a sample of population (30) patients, 15 patients fixed by interference screws and 15 patients fixed by press fit technique, and after collection of the data and statistical studies, we compare between the 2 techniques and between this study and similar studies, we found that the results are comparable in both groups (with some superiority of press fit technique in some clinical aspect ) in addition to the advantages of press fit technique that mentioned formerly, but from our clinical point of view this technique was technically demanding and has a less steep learning curve other than the ordinary well-known techniques in ACL reconstruction. So we have to consider the press fit technique is one of the options in ACL reconstruction, but need more training and practice. _uأن قطع الرباط الصليبى الأمامى هو من اكثر الاصابات التى تصيب الركبة خصوصا عند الرياضيين وقد تم تطوير العديد من الطرق الجراحية لاعادة بناء الرباط الصليبى الأمامى . اختلفت انواع الرقع الصليبية ما بين رقع صناعية او طبيعية سواء من جسم المريض نفسه ) رقعة ذاتية (او من اجسام اخرى، و قد وجد ان الرقع الذاتية افضل فى النتائج و كان افضل الرقع الصليبية هى الاوتارالخلفية للركبة . اختلفت طرق التثبيت للرقعة الصليبية ما بين التثبيت بمسامير قشرية ،مسامير تداخلية، شرائح صغيرة خيوط او دبابيس.و تعتبر طريقة التثبيت بالمسامير التداخلية هى الطريقة القياسية و اللتى تستخدم كمقياس للمقارنة بطرق التثبيت الاخرى، و لكنها لها عيوبها الخاصة بها و اللتى تختلف ما بين ، كسر المسامير، وجود جسم غريب بالركبة،صعوبة اجراء عمليات الرباط الصليبى المرتجع، صعوبة اجراء الفحص بالاشعة المقطعة او الرنين المغناطيسي بعد العملية، بالاضافة الى التكلفة المادية. و لقد تم تطوير طريقة طبيعية لتثبيت الرقعة الصليبية دون الحاجة لوضع اجسام غريبة بالمفصل ، وذلك عن طريق استخدام رقعة عظمية ذاتية من المريض نفسة و يتم تثبيتها على الرقعة الصليبية بطريقة الحشر. و نحن فى هذة الدراسة بصدد المقارنة بين الطريقة القياسية) طريقة المسامير التداخلية( و بين طريقة التثبيت بالرقعة العظمية بطريق الحشر و ستكون المقارنة من الناحية السريرية لمعرفة ما اذا كانت هذة الطريقة مفيدة و فعالة مقارنة بالطريقة التقليدية،حيث ان هذة الطريقة الجديدة تتميز بكثير من المميزات مثل انها طبيعية لا يوجد اجسام غريبة، سهولة عمل عملية الرباط الصليبى المرتجع ، لا تتسبب فى توسيع النفق العظمى للرباط، و لا تؤثر على الاشعة المقطعية او الرنين المغناطيسي بعد العملية ، بالاضافه انها غير مكلفة. ) قد تمت هذة الدراسة على عينة من المرضى تتكون من ثلاثون مريض (30) ، تم تثبيت خمسة عشرة(15) حالة بطريقة المسامير التداخلية ذاتية الذوبات ، و خمسة عشرة حالة )15 (تم تثبيتها بطريقة حشر رقعة عظمة ، وبعد جمع البيانات و عمل الدراسات الاحصائية الازمة و المقارنة بين الطريقتين و كذلك بين هذة الدراسة ودراسات مشابهة تبين ان النتائج متقاربة جدا بين الطريقتين) و ان كانت طريقة حشر الرقعة العظمية تتفوق فى بعض النواحى( ولكنها علي الجانب الاخر ومن خلال الممارسه العمليه لها فانها تعد اصعب نسبيا من الطريقه العاديه وهي المسامير ذاتيه الذوبان وذلك لانها لا تحظي بمنحني عالي من التدريب ، مما يدعونا لاعتبار طريقة التثبيت بحشر الرقعة العظمية احد الخيارات المتاحة فى عمليات اعادة بناء الرباط الصليبى الا انها تحتاج الي التدريب والممارسة العملية بمنحني أعلي .
