520 _aBackground: Hepatocellular carcinoma (HCC) is a major health problem in Egypt. It is third most common cause of cancer deaths worldwide. Both the immune response to hepatitis viral infection and oxidative stress are involved in the pathogenesis of HCC. Objective: To observe the relation between GSH as a marker of oxidative stress and polymorphism of GSTP1 gene which catalyzes conjugation of reduced form of glutathione (GSH) to xenobiotic substrates for the purpose of detoxification. Methods: The patients were recruited from the medical oncology department, National cancer institute, Cairo university as well as the National liver institute Shebein Elkawm Elmonofeya over a period from November 2014 to August 2016. The methods we used were Ellman's colorimetric method for plasma GSH determination and GSTP1gene polymorphism determination samples of 99 adult patients with HCC. Results: The study revealed no significant relation between GSTP1gene phenotype and plasma level of GSH in HCC patients. Prevalence of mutant GSTP1 was higher among the HCC patients compared to the normal control group. There was no significant difference between the patient and control groups as regards the plasma GSH levels (p=0.152) besides its low sensitivity (38%) and specificity (53%)
