520 _aAim of the work: To assess the added value of DCE-MRI as a supplemental imaging modality to mammography in women with increased risk of developing breast cancer. Patients and methods: the study included 105 eligible female patients with increased risk of developing breast cancer(i.e. positive family history of breast cancer of first degree relative, personal history of breast cancer and heterogeneously dense mammary parenchyma ), full field digital mammography(FFDM) and DCE-MRI were performed, the findings were evaluated independently on both mammography and MRI scanning and final BI-RADS classifications was given ineach modality and compared to histopathology results as the golden standard. Results:In total, 105 women were enrolled in the study, the mean age of this study population is 45 years.Among the studied cases, regardless to a specific risk factor, 47/105 (44.7%) proved breast cancercases with histopathology (38 IDC,6 DCIS and 3 cases ILC), mammography detected 35/47 (74.5%) of these lesions and MRI scanning detected 45/47 (95.7%) cases, this giving an overall sensitivity of the MRI up to (95%)in this study in comparison to mammographic sensitivity (74% )while the specificity was(94.83%) and (87.93%)respectively,When comparing the mammography and DCE MRI findings within each risk factor group, the DCE MRI reported higher sensitivity than mammography (MRI study for patients with heterogeneously dense parenchyma reported (93.33%) sensitivity and (97.62%) specificity while the mammography reported significantly lower sensitivity of (66.67%) and specificity (92.86%), with significant P {u2013}value 0.001)
