520 _aBackground: postmenopausal bleeding is common patient complaint. The 1ry goal of investigation is to exclude endometrial carcinoma. Aim of work: to evaluate the diagnostic accuracy of 3D endometrial volume using VOCAL software and 3D power Doppler (3D-PD) endometrial vascular indices in distinguishing between benign and malignant endometrial lesions among menopausal women complaining of postmenopausal bleeding. Patient and methods: 100 menopausal patients complaining of postmenopausal bleeding were included. Patients were examined by 2D TVUS (to measure endometrial thickness) followed by 3D TVUS equipped with VOCAL software and 3D PD (to measure endometrial volume and endometrial vascular indices). Following D&C, all specimens were examined histopathologically and the results were corelated with corresponding ultrasound findings (ET, EV, VI, FI and VFI). Results: malignant lesion had a significantly larger EV (9.05 vs 3.48 cm3) compared to benign ones. They also had a significantly higher VI (7.15 vs 2.51%), FI (24.16 vs 17.08) and VFI (1.741 vs 0.469). The sensitivity, specificity and accuracy of EV ({u2265} 5.86 cm3) and VI ({u2265} 5.43%) in detecting malignancy were 94.1%, 85.5% and 87%, respectively. The sensitivity, specificity and accuracy of FI ({u2265} 21.27) and VFI ({u2265} 1.162) in detecting malignancy were 94.1%, 91.6% and 92%, respectively
