520 _aBackground: Cisplatin, a potent anticancer, is known for its nephrotoxicity. Risk may increase with increased cumulative dose, bolus dosage and other nephrotoxic agents associated. Clinical pharmacist has important role in adverse drug events management as well as detecting risk patients. Aim: This study aims to identify incidence of cisplatin nephrotoxicity and associated risk factors and early nephrotoxicity detector(s) within pediatrics at Children's Cancer Hospital. Identification of early toxicity detector(s) of nephrotoxicity by using early therapeutic cycle(s) exposure might be valuable predictor of late nephrotoxicity, so that severe patient intoxication could be prevented. Methods: 51 patients, age less than or equal to 18 years old, received cisplatin-based chemotherapy at doses (20-120 mg/m2/cycle), monitored and scored for nephrotoxic events throughout the chemotherapy course, using NCI CTCAE, then correlated to patients' demographics, cisplatin dosage, and associated nephrotoxic drugs using logistic regression. Cramér{u2019}s V was used to test the degree of association between late nephrotoxicity and earlier cycles' toxicity, and to constitute a link between individual nephrotoxicity events and overall patient exposure score
