520 _aLower pole breast support in implant based breast reconstruction using the a Dermal flap is a new surgical Technique at the National cancer institute, with many challenges faced and surgical technicalities required to achieve an implant pocket that will offer adequate support to the implant, a good cosmetic result, and avoid patient complications.This study was conducted at the Breast Surgery unit, National Cancer Institute, Cairo University, during the period from January 2017, till March 2019, where the aim was to assess the feasibility of this surgical technique, the associated complications and aesthetic outcome at the NCI. The study included 16 patients; 12 patients with pathologically proven breast cancer, and 4 patients with BRCA1 positive gene, for whom the procedure was done on both sides, making a total of 20 procedures of breast reconstruction. On the day of surgery, markings were done based on an wise pattern. De-epithelialization of the lower skin flap, then mastectomy carried out in the conventional mastectomy plane. The lower border of the pectoralis major muscle is partially released from its origin, and sutured to the dermal flap in order to create the implant pocket. On follow up after surgery, all complications were reported and managed. Post-Operative photos were taken in the Anteroposterior and lateral views, and the BREAST-Q {u2122} was filled. Patient satisfaction and quality of life in the postoperative period were determined using the BREAST-Q {u2122} questionnaire. Post-operative images were assessed regarding cosmetic outcome using BCCT.core.3.1 software
