520 _aBackground: MiRNAs has a role in physiological processes, including cell proliferation, differentiation, apoptosis and metabolism consequently their dysregulation appears to be involved in various disorders, including metabolic diseases such as diabetes. Adiponectin is the most abundant peptide hormone secreted by adipocytes. It has become a biomarker for obesity-related diseases as metabolic syndrome, type 2 Diabetes mellitus, and cardiovascular disease.Aim of the work: The aim of this work was to assess correlation between exosomal miRNA 326 and adiponectin in diapetic patients.Subjects and Methods: One hundred diabetic patients and fifty healthy subjects were included in the study. They were classified into three groups: Group (I): Fifty apparently healthy volunteers as control subjects. Group (II): Fifty patients with type I diabetes. Group (III): Fifty patients with type II diabetes. MiRNA was extracted from isolated exosomes samples of both the diseased and control groups . MiRNA 326 quantitative gene expression was assessed by real-time polymerase chain reaction. Adiponectin and Insulin were assessed by ELISA. Correlation between miRNA326 gene expression and adiponectin level was analysed statistically.Results: There was significant increase in HOMA level in type II diabetics compared to control subjects and type I diabetic patients. Severe insulin resistance was most significantly occurring in type II diabetes, significant increase in miRNA 326 expression in type I and type II diabetic patients compared to control subjects, significant decrease in adiponectin level in type I and type II diabetic patients compared to control subjects and significant decrease in adiponectin level in type II diabetes patients when compared to type I diabetic patients
