520 _aBackground: High-flow nasal cannula (HFNC) therapy is a noninvasive respiratory support which is a simple alternative to other forms of pediatric respiratory support modalities . Objectives: In the present study, we aimed to assess the relation between success and failure of the HFNC in relation to primary diagnosis, associated systems affection, PRISM III, SpO2/FIO2 (S/F) ratio and length of hospital stay. Patients and methods :The study included retrospectively medical records of 139 children using HFNC in PICU in Abo El Reesh tertiary hospital between the period June 2017 to December 2019.Results:We found significant association between HFNC success as initial O2 support and the diagnosis, higher S/F ratio, lower PRISM III and lower length of PICU stay. Regarding its use in postextubation, success was statistically associated with the diagnosis of surgical causes and cases with no underlying system affection , higher S/F ratio, lower PICU stay, while failure was statistically significant with the diagnosis of non-operated congenital heart disease patients. Mortality was lower with the success of HFNC. Conclusion : Success or failure of HFNC as an initial respiratory support appears to depend on the cause of the respiratory failure , severity of illness and underlying system affection
