520 _aBackground: Primary dysmenorrhea was defined as the contractions of the uterine muscles of the uterus which induce a local ischemia. During a woman's menstrual cycle, the endometrium thickens in preparation for potential pregnancy. After ovulation, if the ovum is not fertilized and there is no pregnancy, the built-up uterine tissue is not needed and thus shed. Purpose of this study: was to investigate the effect of shock wave on primary dysmenorrhea Methods: A randomized controlled trial were carried out on fifty adult female with regular menstrual cycle who suffered from primary dysmenorrhea, with mean age 21.16 ± 1.77 assigned into two equal groups, Group "A" received shock wave therapy (5000 shock/session, 10Hz and EFD 0.178 mJ/mm2) plus dietary modification for one session prior the estimated day of menstrual cycle and 3 sessions for the first 3 days of menstrual cycle of three consecutive period, Group " B " received dietary modification for three months. Assessment of pain was determined by numerical rating Scale (NRS) twice before and after treatment procedure and the objective measurement of degree of menstrual pain Prostaglandin F2 Alpha were used, The sample was taken (on the 2nd day of menstruation before treatment);and on the 2nd day of menstruation cycle after 3 menstrual cycles treatment. Results: Showed significant changes in all measured variables as a result of using shock wave for primary dysmenorrhea. Also results proved that SW was more effective than dietary modification
