520 _aDespite different protective measures focusing on the prevention of dental caries. However, dental caries remains a significant public health problem and is one of the most common chronic, infectious and multifactorial disease in the world. This study aimed to evaluate the effect of different preventive regimens using different toothpastes on caries risk by using cariogram and DMF scores of high caries risk patients.Three-armed, parallel-group, randomized, controlled trial study was conducted on 66 participants with high caries risk. Participants were divided randomly into three groups according to the test regimen(n=22), where (A1) participants were exposed to a regimen including dual zinc plus arginine based toothpaste (Colgate total), (A2) participants were exposed to a regimen including zinc based toothpaste (Crest complete), and (A3) control group where participants were exposed to a regimen including fluoride based toothpaste (Signal). The study was carried over a period of one year, assessment of cariogram and DMF scores were done in four visits: first visit (baseline: T0), second visit (after 3 months: T3), third visit (after 6 months: T6) and final visit (after 12 months: T12). The results showed that caries prevention regimen using dual zinc and arginine based toothpaste was statistically and clinically significant higher in altering patient{u2018}s caries risk when compared with regimen using zinc based toothpaste. These regimens were shown to be significantly superior to fluoride based toothpaste at all time intervals.The conclusion is preventive regimens containing different toothpastes are positively influencing caries prevention in high caries risk patients. Dual zinc plus arginine based toothpaste is a superior caries preventive regimen in high caries risk patients
