Background: Unstable surface training is beneficial for children with spastic
diplegic cerebral palsy who had gait and balance problems. The quad spring board
and platform could provide an unstable surface during gait training for those
children. Aim of the study: The purpose of the current study was to investigate if
there is an effect of the quad spring board walkway and platform swing on gait
pattern in children with spastic diplegia. Materials and methods: Thirty spastic
diaplegic cerebral palsy children of both genders, ranging in age range from 4 to 7
years participated in the study. The degree of spasticity ranged from mild up to
moderate (1 or 2) according to the Modified Ashworth Scale and they were at level
I or II on the Gross Motor Function Classification System. They were divided
randomly into two equal groups (control and study). The control group received
traditional physical therapy program, and the study group received the same
program given to the control group in addition to gait training on the quad spring
board walkway integrated into the dynamic platform swing walkway. All subjects
were assessed before and after three months of treatment using GMFM-88 to
determine their growth motor function and tracker video analysis and modeling
tool to determine lower limbs‘ joints angle. Results: There was significant increase
in all mean values of the measured variables when comparing the pre and post
treatment results of each group. Comparing the post treatment mean values of both
groups revealed significant improvement of all the measured variable in favor of
the study group except for the ankle joint. Conclusion: Quad spring board
walkway combined with platform swing have significant effect on gait pattern in
children with spastic diplegia which may be due to stimulation of the vestibular
system stimulation.
شارك فى الدراسة 30 طفلا مصابا بالشلل الدماغي من كلا الجنسين أعمارهم بين 4 و7 سنوات مع التشنج من خفيف إلى معتدل (1 أو 2) وفقا لمقياس أشوورث المعدل وكانوا في المستوى الأول أو الثاني على نظام تصنيف الوظائف الحركية الإجمالية. تم تعيينهم عشوائيا في مجموعتين متساويتين. تلقت المجموعة (أ) برنامج العلاج الطبيعي التقليدي، وتلقت المجموعة (ب) نفس البرنامج التقليدي بالإضافة إلى التدريب على المشي على ممر رباعي الزنبرك مدمج في ممر تأرجح المنصة الديناميكي. تم تقييم جميع الأشخاص قبل وبعد الدراسة باستخدام GMFM-88 (أجزاء الوقوف والمشي والجري والقفز) وباستخدام أداة تحليل ونمذجة فيديو التتبع. لوحظ وجود هناك زيادة ملحوظه احصائيا في جميع المتغيرات المتوسطة ل GMFM-88 وزوايا مفاصل الورك والركبة في مرحلة منتصف الوقوف باستثناء مفصل الكاحل لصالح مجموعة الدراسة (p > 0.05).
