In this study, twenty five cases with different patterns of maxillofacial fractures that are delayed in management due to many factors most commonly is multisystem trauma. Fractures were classified according to CT3D findings. The plan of management was aiming to setup a protocol for management of delayed maxillofacial fractures and determining the period after which the fracture is called delayed. The timing of operation is important and should be early within 5-14 days Evaluation is done by clinical evaluation, CT3D, panoramic view and plain X-ray. Treatment of delayed maxillofacial fractures was done by different modalities like osteotomy and refracturing when fractures are malunited and by rib grafting in deficient bones also by titanium mesh in orbital fractures. No complications were observed in 24 cases while mild enophthalmos in one case
