Delay in children vaccination is one of the common problems that encountered by many countries especially Egypt. It exposes the child’s health to severe diseases. Determinants of delay of vaccination must be taken into consideration such as mother age, child weight at birth, etc. This study focuses on studying the main factors affecting the time from birth to the exact date of receiving the first dose of measles, mumps and rubella (MMR) vaccine.
Survival analysis is one of the primary statistical methods for analyzing time-to-event data. Hazard rate function is one of the most important components in survival analysis. The hazard rate performance can be changed suddenly after a certain point of time or after a covariate reaches a certain threshold which refers to the existence of a change point. It is often difficult to detect and analyze the effect of such changes using the traditional procedures as stratifying the data set into different strata and analyzing each one separately (before and after the change point). Also, ignoring the effect of such changes may lead to inaccurate results.
A graphical method is used to detect the threshold where the change point occurs (if existed) in the covariates which changes the performance of the hazard of delay of MMR vaccine. Cox proportional hazards model with change point (extended version) is adopted to explore the associated factors of delay of MMR vaccination among children under five using 2014 Egyptian demographic health survey (EDHS) data. It is found that there is a significant change in the hazard of delay in vaccination after the maternal age achieved 34 years old. In addition, higher vaccination coverage, and increasing number of antenatal visits, increasing in the number of children under five, the older the maternal age, lower birth order and the lower number of household members contribute to decrease the hazard of delay in MMR vaccine. يعد تأخير تطعيم الأطفال من المشاكل الشائعة التي تواجهها العديد من الدول وخاصة مصر. لانه يعرض صحة الطفل لأمراض خطيرة. لذلك يجب مراعاة محددات تأخير التطعيم مثل عمر الأم ووزن الطفل عند الولادة وما إلى ذلك. تركز هذه الدراسة على دراسة العوامل الرئيسية التي تؤثر على الوقت منذ الولادة وحتى التاريخ الدقيق لتلقي الجرعة الأولى من (MMR)الحصبة والنكاف والحصبة الألمانية لقاح. تحليل البقاء على قيد الحياة هو أحد الأساليب الإحصائية الأساسية لتحليل وقت الحدث. تعد وظيفة معدل الخطر أحد أهم المكونات في تحليل البقاء على قيد الحياة. يمكن تغيير أداء معدل الخطر فجأة بعد نقطة زمنية معينة أو بعد أن يصل المتغير المشترك إلى حد معين يشير إلى وجود نقطة تغير. غالبًا ما يكون من الصعب اكتشاف وتحليل تأثير هذه التغييرات باستخدام الإجراءات التقليدية مثل تقسيم مجموعة البيانات إلى طبقات مختلفة وتحليل كل واحدة على حدة (قبل نقطة التغير وبعدها). كما أن تجاهل تأثير هذه التغييرات قد يؤدي إلى نتائج غير دقيقة. تُستخدم الطرق البيانية لتحليل أداء مخاطر تأخير التطعيم بلقاح الحصبة والنكاف والحصبة الألمانية واكتشاف الحد حيث تحدث نقطة التغير (إن وجدت). تم اعتماد نموذج الخطر النسبي كوكس مع نقطة التغير (نسخة موسعة) لاستكشاف العوامل المرتبطة بتأخير التطعيم ضد الحصبة والنكاف والحصبة الألمانية بين الأطفال دون سن الخامسة باستخدام بيانات المسح الديموغرافي والصحي لمصر2014 . وجد أن هناك تغيرا كبيرا في خطورة تأخير التطعيم بعد بلوغ الأم 34 عاما. بالإضافة إلى ذلك ، يسهم انخفاض ترتيب الولادة ، وزيادة تغطية التطعيم ، وزيادة عدد زيارات ما قبل الولادة ، وزيادة عدد الأطفال دون سن الخامسة ، وكبر الأم ، وانخفاض عدد أفراد الأسرة في تقليل خطر التأخير في لقاح الحصبة والنكاف والحصبة الألمانية.
