Background: Worldwide attention for post-surgical scar as a visible endpoint of healing. Potential evidence regarding vacuotherapy as an alternative therapy, and well-known shock wave therapy for their therapeutic benefits on surgical scars. Purpose: To determine efficacy of vacuotherapy versus shock wave therapy on post-surgical scars.
Method: Forty patients of both genders with immature surgical scars <6 months from Kafr Shokr Specialized Hospital, and Dermatology Hospital, Banha, participated in this study and they were 20-45 years old, free from any post-surgical complications. They were randomly allocated into two equal groups in numbers; Group A received vacuotherapy 3 sessions per week; and Group B received extracorporeal shock wave therapy one session every two weeks, from June 2023 to March 2024. Evaluation involved scar pliability, vascularity, pigmentation and height using Vancouver scar scale and objective scar pliability and elasticity using modified schiotz tonometer. Statistical analyses with significance level 0.05 level.
Result: Unsignificant differences revealed at baseline analysis. Both groups had revealed a significant decrease in Vancouver scar scale reported values, and a significant improvement in modified schiotz tonometer reported values post-treatment. While, there was significant improvement in group I compared with group II post-treatment in term of scar pliability, vascularity, pigmentation, height, and elasticity.
Conclusion: Both vacuotherapy and shock wave therapy are valuable. Shock wave therapy was superior in terms of scar pliability, vascularity, pigmentation, height, and elasticity. Therefore, shock wave therapy is recommended to manage immature surgical scars with functional restrictions management الخلفية العلمية: تزايد الإهتمام العالمي بندبات ما بعد الجراحات أضحي نقطة إنطلاق لشفاء متكامل. فمع تنامي دلالات استخدام العلاج بالتفريغ الهوائي كعلاجاً بديلاً، وكذلك العلاج بالموجات التصادمية لتعاظم فوائدها العلاجية لعلاج الندبات الجراحية.
الغرض: تم إجراء الدراسة لتقييم مدي فعالية العلاج بالتفريغ الهوائي مقابل العلاج بالموجات التصادمية علي الندبات الجراحية. إجراءات الدراسة: أجريت الدراسة علي أربعين مريضًا من كلا الجنسين يعانون من ندبات جراحية غير مكتملة ما بعد الجراحات قبيل ستة شهور. أجريت هذه الدراسة في العيادات الخارجية، مستشفي كفر شكر التخصصي ومستشفي الجلدية التخصصي؛ بنها- محافظة القليوبية؛ مصر. تم تقسيم المرضي إلى: المجموعة الأولي تلقت العلاج بالتفريغ الهوائي - والمجموعة الثانية تلقت العلاج بالموجات التصادمية علي مدي ثمانية أسابيع متتالية مع التقييم قبل وبعد الدراسة باستخدام مقياسي بمقياس فانكوفر للندبات وشدة التوتر شيوتز المعدل وتبعه تحليل إحصائي تفصيلي. النتيجة: لم يتبين وجود فروقاً ذات دلالة إحصائية بأعمار مجموعتي الدراسة (<٠٥.٠). لا يوجد إختلافات إحصائية قبل بدء العلاج بمقياس فانكوفر للندبات ومقياس شدة التوتر شيوتزالمعدل بين مجموعتي الدراسة قبل البدء بالدراسة، ولكن تبين وجود إختلافات ذات دلالة معنوية مرتفعة بمقياسي فانكوفر للندبات وشدة التوتر شيوتز المعدل (>٠٥.٠) بعد الإنتهاء من العلاج. الخلاصة: العلاج بكلا من التفريغ الهوائي والموجات التصادمية لمدة ثمانية أسابيع متتالية ذا فعالية علاجية مع الندبات الجراحية؛ مع تفوق نوعي للعلاج بالموجات التصادمية وفقا لمعايير التقييم لمدي تشبعها بالتغذية الدموية، ومعدلات التحسن اللوني وكثافة الندبات ومرونتها..
