Background:
Early pregnancy loss remains the most common complication of
the first trimester. Accurate prediction of pregnancy outcome is of great
clinical importance. Transvaginal ultrasound provides reliable markers in
early pregnancy, including gestational sac diameter (GSD), yolk sac
diameter (YSD), crown-rump length (CRL), and fetal heart rate (FHR). In
addition, corpus luteum (CL) Doppler indices may reflect luteal blood
flow and progesterone production, and thus help predict the fate of
pregnancy.
Aim of the Work:
To evaluate the role of early pregnancy ultrasound parameters
including corpus luteum Doppler in predicting first-trimester pregnancy
outcome in spontaneous pregnancies.
Patients and Methods:
This prospective observational study was conducted on 120
pregnant women attending Kasr Al-Ainy Hospital, Cairo University,
between January and June 2025. All participants were examined by
transvaginal ultrasound at 6-, 8-, 10-, and 12-weeks’ gestation.
Parameters assessed included GSD, YSD, CRL, FHR, and corpus luteum
Doppler indices (RI and PI). By 12-13 weeks, patients were classified
into two groups: ongoing pregnancy or miscarriage. Data were analyzed
and compared between both groups using appropriate statistical tests.
Results:
CRL, FHR, and CL Doppler indices (RI and PI) showed
statistically significant differences between ongoing and miscarriage
groups, with higher predictive accuracy than GSD and YSD in early

weeks. ROC curve analysis demonstrated that CL Doppler indices had
good discriminative ability, with sensitivity and specificity exceeding
75% for miscarriage prediction.
Conclusion:
Early pregnancy ultrasound parameters, particularly CRL and
FHR, together with corpus luteum Doppler indices, are valuable non-
invasive predictors of first-trimester pregnancy outcome. Incorporating
these parameters into routine early pregnancy assessment may improve
prediction and allow timely counseling and management. يمثل الفحص بالموجات فوق الصوتية في الحمل المبكر اداة اساسية لتاكد حيوية

معامل المقاومة ومعامل النيضية التي تعد عنصرا حاسما للحفاظ علي الحمل خلال الثلث الاول
وتشير الاضطراابات في هذة الموشيرات الي ذيادة احتمالية فقدان الحمل المبكر مما يؤكد قيمتها
في التنبؤ بمؤشر الحمل خلال الثلث الاول اجريت هذة الدراسة بجامعة القاهرة وشملت 120
سيدة لديهن حمل مفرد حي بين الاسبوع السادس والثاني عشر من الحمل تضمنت معاير
الادراج السيدات اقل من 40 عاما مع تائكيد وجود حمل رحمي حي بينما استبعدت الحالات
المصابة بحمل خارج الرحم او الحمل المتعدد او العلة الكيسية العنقودية او العيوب الرحمية
او الامراض الطبية المزمنة الكبري او الاجهاض في الزيادة الاولي تم اجراء فحص بالموجات
فوق الصوتية المهبلية اسبوعيا لقياس الصطر الكيسي الحمليقطر كيس الصفار طول الجنين
من الرئس الي المقعدة ومعدل ضربات القلب الجيني بالاضافة الي دوبلر الجسم الاصفر
واعيد تقييم النتائج في الاسبوع الرابع عشر لتاكد استمرار الحمل او تشخيص الاجهاض
