Background: Gastrointestinal endoscopy (GIE) is a widely performed procedure for the diagnosis and treatment of gastrointestinal diseases. The risk of infection for patients undergoing these procedures has been progressively reduced by the implementation of infection control guidelines and adequate training of nursing staff. Therefore the current study was conducted to assess the selected determinants of nurses compliance to infection control guidelines in gastrointestinal endoscopy units. Methods: A convenient sample of 30 nurses working at three selected endoscopy units at three of the biggest teaching hospitals in Cairo Governorate were recruited to fulfil the aim of this study. A descriptive, exploratory research design was utilized. Three tools were used to collect the data: (i) semi structured interview questionnaire, (ii) observational checklist for nurses practice and (iii) microbiological report.Results:76.7% of the subjects were females with a mean of age 39.8± 7.7 years. All of them hold secondary school nursing diploma. 73.3% had un-satisfactory level of knowledge in accordance with the national guideline for the endoscope reprocessing. More than 90%of them had unsatisfactory performance level in all the eight reprocessing steps, and 76.66% had unsatisfactory level in the storage step. There was a statistically significant difference (p < 0.05) across the three endoscopy units. Out of 44 endoscope swabs, 27.27% were positive for Pseudomonas Aeruginosa and Staph Aureus before and after using the endoscope suggesting presence of biofilms,
