Obejective the aim of this work is to compare between the results of bilateral pedicle screws fixation with posterolateral intertransverse fusion versus unilateral pedicle screws with interbody fusion in management of single level lytic spondylolithesis in lumbar spine. Methods: This is a controlled randomized study where randomization was planned to perform bilateral pedicle screws fixation with posterolateral intertransverse fusion in odd number patients while unilateral pedicle screws with interbody fusion were performed in even number patients. Results: The median age at presentation in this study was 44,9 years, there were four males and ten females, postoperative pain improvement was better in unilateral group . Hospital stay, blood loss, and postoperative complications were less in unilateral group
