Aim: The aim is to study the role of multifocal Electroretinogram (mfERG) and central visual field test in the evaluation of macular function in patients with diabetic clinically significant macular edema (CSME) and correlate their measurements with central foveal thickness (CFT) as measured by Spectral domain optical coherence tomography (SD-OCT) to elucidate the possible value of mfERG and central visual field in management of diabetic maculopathy. Methods:Thirty eyes were included in this Prospective Cohort interventional study. These thirty eyes belonged to diabetic patients with diabetic macular edema (DME). All subjects were subjected to complete ophthalmic examination, including a review of medical and family history, visual acuity testing by Snellen fraction, and slit-lamp biomicroscopy of anterior and posterior segment. Retinal imaging usingFFA and SD-OCT was done to all patients to obtain the CFT, confirm the clinical diagnosis and detect exclusion criteria e.g. ischemic maculopathy. Afterwards, all included eyes were examined using the mfERG using standard protocols to determine the P1 wave amplitude and implicit time from the foveal region, in addition to central VF to detect the presence of central scotoma and its threshold. Results: P1 amplitude didnt show correlation with improvement of the BCVA and CFT, P1 implicit time showed significant improvment in ring 2 of the mfERG after treatment with ranibizumab. MD and PSD showed significant improvement after treatment with ranibizumab
