Objectives: The aim of this study is to evaluate the retinal and choroidal microvascular changes by OCTA in eyes with sarcoid and tuberculous posterior uveitis and associate these changes with their corresponding FFA findings and also with the disease type, treatment and activity. Methodology: The study was conducted on 30 eyes of 19 patients with sarcoid and tuberculous posterior uveitis. They were selected from uveitis subspeciality clinic at Kasr El Ainy Hospital, Cairo University. FFA and OCTA images (6x6mm) were acquired at Kasr El Ainy Laser Unit and BDEC. Qualitative and quantitative changes by OCTA and FFA were analyzed and compared. Results: A total of 30 eyes of 19 patients, 16 patients were females and 3 patients were males. TB eyes were 11 and sarcoid eyes were 19. The mean age was 42 ± 13 SD. The mean BCVA was 0.391 ± 0.270 SD. By OCTA, 100% of eyes had areas of capillary non perfusion / hypo perfusion, capillary changes were found in 50% of eyes, choroidal voids were detected in 36.7% of eyes, disorganizations of the superficial and deep capillary network were found in 30% of eyes, and intraretinal cystoid spaces were found in 20% of eyes. Also, 86.6% of eyes had ischemia in the whole enface image (mean VD <50%) in the SCP and DCP. The mean whole image capillary vessel density in the SCP was 45.3% ± 3.9 SD and in the DCP was 43.1% ± 5.5 SD. On comparing the OCTA changes between the TB group and the sarcoidosis group, our study showed that there was statistically significant difference between them in the vascular density of the DCP; being lower in the TB group
