Cancer is the second most common cause of death among children aged 1 to 18 years in the United States, surpassed only by accidents.Treatment-related neurotoxicity is a potentially life threatening clinical condition that can represent a diagnostic challenge. Differentiating diagnoses between therapy associated brain injury and recurrent disease can be difficult, and the immediate recognition of neurotoxicity is crucial to provide correct therapeutic management, ensuring damage reversibility.Purpose: Is to make radiologists aware of chemotherapy neurotoxicities and early detection of it using MRI & its early management to prevent permanent damage. Patients and methods: We evaluated 63pediatric patients with suspected neurotoxicity related to chemotherapy treatment. MR examinations were performed with a 1.5-T system. MRI data was collected and correlated with clinical data. Results:41 patients diagnosed as PRES with 8 patients had typical MRI criteria and 33 patients have atypical criteria, 16 patients diagnosed as cerebral venous sinus thrombosis with MRV is the most important sequence to diagnose CVST and finally 6 patients diagnosed as methotrexate neurotoxicity with DWI is the most important sequence for early diagnosis
