AIM: The aim of this study is to evaluate implant stability, radiographic bone changes in terms of horizontal and vertical bone loss and pink esthetic score in immediate implant installation and immediate provisionalization with and without grafting the jumping gap with a mixture of 1:1 autogenous and xenograft particulates. Methodology: Twenty- four patients with non-restorable single tooth in the maxillary anterior region indicated for extraction were enrolled in this study, all received immediate implant placement with immediate temporization.These patients were randomly selected from the Outpatient Clinic of the Oral and Maxillofacial department of Cairo University and equally divided into study and control group.The twelve patients in the study group received a treatment of grafting the jumping distance created after implant placement.This graft was a mixture of 1:1 xenograft to autogenous bone particulates.The autogenous bone was collected by low-speed drilling from within the socket.The twelve patients of the control group did not receive the graft. Outcome measures were: Secondary Implant Stability, amount of marginal bone loss (Horizontal and Vertical), PES after 6 months. Results: The mean implant stability quotient for study group was 68.5± 3.1 and increased to 77.6 ± 3.7 after 6 months, while for the control group it was 66.3± 5.4 and increased to 74.2 ± 4.5 after 6 months. Mean results regarding horizontal bone loss at platform level for study group was 0.09 ± 0.06 and was 0.26± 0.27 for control group, at mid-implant level was 0.5± 0.26 for study group and 0.43± 0.48 for contro1 group, at apex level mean results were 0.42 ± 0.25 for study group and 0.59± 0.52 for control group
