Uterine fibroids are the most common benign tumors that worsen women's health during their reproductive age. However, its negative impact on the women's quality of life, there are few conservative therapeutic options to control it. This study was conducted to investigate the effect of ultrasound waves subserosal and intramural. 46 leiomyomas were divided into 30 leiomyomas as experimental group and 16 leiomyomas as a control groups, each group was subdivided into another 3 groups (A1), (B2), and (C3), received ultrasound (continuous,1 MHZ, 2Watt/cm2) for 15, 30, and 45 minutes respectively in experimental group, while they were not exposed to ultrasound in control group. All sizes of leiomyomas were measured using the fluid displacement method before and after 15, 30, 45 minutes in both groups. The sizes of uterine leiomyomas showed a significant decrease (P-value=0.0001) in all groups (A), (B) & (C) of experimental group after the application of ultrasound, also it showed a significant decrease (P-value=0.049), (P-value=0.034) (Pvalue= 0.011) in group (A), (B) & (C) of experimental group respectively after the application of ultrasound when it compared with control group beyond 15, 30, 45 minutes after the leiomyomas excision. While, the sizes of leiomyomas showed a non-significant difference in-between groups (A), (B) & (C) after the application of ultrasound in experimental group (P=0.99) and beyond 15, 30, 45 minutes after the excision in control group (P=1.00). Histopathological evaluation depending on hematoxylin and eosin (H&E) stains and (Caspase-3) stain were performed after ultrasound application for experimental group and for only 6 leiomyomas selected randomly from control group. In hematoxylin and eosin (H&E) stains after the application of ultrasound, apoptosis was observed in 100% of leiomyoma samples in group (A), (B), and (C)
