| Summary, etc. | Background: Levosimendan has anti-ischaemic effects, improves myocardial contractility and increases systemic, pulmonary and coronary vasodilatation. The present study investigates the perioperative hemodynamic effects of a prophylactic infusion of levosimendan in high-risk mitral valve surgery patients with left ventricle dysfunction, and compares short-term clinical outcomes with a control group in which levosimendan weren’t used.<br/>Patients and methods: A prospective randomized clinical study was performed in 100 patients with high risk mitral valve surgery with left ventricular dysfunction and pulmonary hypertension. In the study group, patients will receive Levosimendan infusion at a dose of 0.1 mcg/kg/min after the induction of anasethia while in control group levosimendan will not be used. Additional inotrope and/or vasoconstrictor may be used based on hemodynamic parameters. The intraoperative and postoperative data were recorded for each patient in both groups. The hemodynamic measurements were performed at six predetermined time points (0, 1, 6, 12, 24 and 36h postoperatively).<br/>Results: Levosimendan had significantly improved postoperative hemodynamic values. It improved mean arterial pressure at different times postoperative (p < 0.05), heart rate at different times postoperative (p < 0.05). Also levosimendan preserve LV systolic performance postoperatively (PAP: 51.7 ± 6.4, 57.9 ± 8.6, P<0.001*) (EF: 37.1 ± 9.3, 33.4 ± 7.1, P=0.03*).<br/>While there was no significant difference in ICU stay (3.24 ± 1.3, 3.6 ± 1.13, P= 0.14) and mortality between both groups (3 vs. 5, P= 0.46).<br/>Conclusion: Prophylactic levosimendan improved the haemodynamics in high risk mitral valve surgery patients. So Levosimendan seems to be a safe and effective choice for preventing left ventricular failure in high-risk mitral valve surgical patients with LV dysfunction. |

| Summary, etc. | يحتوي عقار الليفوسيمندان على تأثيرات مضادة للاقفار و يحسن من تباين عضلة القلب و يزيد من توسع الاوعية الجهازيه و الرئوية و التاجيه. مما يميزه عن العقارات الاخرى المنشطة لعضلة القلب. تبحث هذه الدراسة في تأثير الاستخدام الوقائي لعقار الليفوسيمندان على علامات الدورة الدمويه المحيطة بالجراحه في مرضى جراحات استبدال الصمام الميترالي عالية الخطوره و الذين يعانون من ضعف في عضلة القلب و ارتفاع ضغط الشريان الرئوى وتقارن هذه النتائج السريريه قصيرة المدى مع مجموعة اخرى لا يتم استخدام عقارالليفوسيمندان فيها. و قد أجريت هذه الدراسة بقسم جراحة القلب والصدر بمستشفى القصر العينى جامعة القاهرة و مستشفى جامعة الفيوم فى الفتره ما بين أكتوبر 2019 ومايو 2021 ، و قد تضمنت هذه الدراسة 100 مريضا يعانون من امراض الصمام الميترالي عالية الخطوره مع ضعف عضلة القلب و اجريت لهم جراحة استبدال الصمام الميترالي. و لقد تم تقسيم المرضى الى مجموعتين متساويتين : المجموعة الاولى التى تم استخدام عقار الليفوسيمندان اثناء الجراحه و عددهم 50 مريضا و المجموعة الثانية التى لم يتم استخدام العقاروعددهم 50 مريضا. و قد تم تقييم حالة جميع المرضى قبل و أثناء و بعد الجراحه مع التركيز على بعض الجوانب مثل: العلامات الحيويه أثناء و بعد الجراحه؛ مدة التنفس الصناعي وإستخدام مقويات عضلة القلب. وحالة المرضى داخل الرعاية المركزه عقب اجراء الجراحه؛ و المده التي قضاها المريض داخل الرعايه المركزه و المستشفى؛ و تأثير ذلك على التكلفه النهائيه للمستشفى و الاداء الانقباضي لعضلة القلب بعد الجراحه و كذلك معدل الوفاه بين المرضى. و كانت النتائج توضح ان الاستخدام الوقائى لعقار الليفوسيمندان يحسن بشكل ملحوظ العلامات الحيويه للمريض بعد الجراحه. و يقلل الحاجة الى رافعات الضغط للمرضى اثناء و بعد الجراحه وكذلك يحافظ على الاداء الانقباضي لعضلة القلب وكذلك أوضحت الدراسة انه ليس هناك تأثير واضح لإعطاء العقار على المدة الكلية لإقامة المريض بالرعاية و المستشفى وكذلك نسبة الوفيات بين المرضى الذين خضعوا للجراحه. و بناء على هذه النتائج ننصح باستخدام عقار الليفوسيمندان في مرضى جراحات الصمام الميترالي عالية الخطوره حيث انه عقار آمن و فعال لتقليل نسبة حدوث متلازمة قصور الناتج القلبي لهؤلاء المرضي. |
