| Summary, etc. | Hypotension with general anesthesia induction a frequent event in routine practice(1). Even a short duration of hypotension may lead to tissue hypoperfusion and predispose to postoperative complications (2). Intra-operative hypotension is associated with renal injury, ischemic stroke, myocardial injury and postoperative mortality (3–7). Underlying hypovolemia is an important and modifiable risk factor for hypotension after anesthetics administration(11,12). Ultrasonographic studies of the inferior vena cava (IVC) (11,16) and the internal jugular vein (IJV) (17,18) for evaluation of intravascular volume status and prediction of hypotension during induction of general anesthesia have been established.<br/>The present study was designed to compare, on ultrasound-based measures, between inferior vena cava and internal jugular vein for prediction of prolonged hypotension during induction of general anesthesia. The study was conducted at Kasr Al-Ainy hospital, Cairo University in Patients undergoing elective non-cardiac surgery under general anesthesia.<br/>One-hundred and sixty three patients were screened for eligibility, 6 patients were excluded for not fulfilling the inclusion criteria, 14 patients were excluded due to poor visualization of IVC, 10 patients were excluded for prolonged airway instrumentation during intubation and 133 patients were included and were available for the final analysis.<br/>Our study demonstrated that ultrasonographic scanning of the IJV change rate preoperatively can predict prolonged post-induction hypotension as well as severe post- induction hypotension and its predictive value is moderate. On the other hand, none of IVC parameters (CI, dIVCmax & dIVCmin) showed a reliable accuracy in prediction of prolonged or severe post-induction hypotension |

| Summary, etc. | يعد انخفاض ضغط الدم مع التخدير الكلي حدثاً شائعاً ؛ وقد يتسبب في أضرار بالقلب والكلى والمخ ومضاعفات قد تصل إلى الوفاة بعد العمليات الجراحية حتى لو كان انخفاض الضغط لفترات قصيرة ؛ كما يعد نقص السوائل بالدم أحد أهم أسباب انخفاض ضغط الدم مع التخدير الكلي والتي يمكن معالجتها ، وقد تم إجراء دراسات على قياسات الوريد الأجوف السفلي والوريد الوداجي الداخلي باستخدام الموجات فوق الصوتية في تقدير كمية السوائل بالدم وفي توقع انخاض ضغط الدم مع التخدير الكلي .تم تصميم الدراسة الحالية لمقارنة قياسات الوريد الأجوف السفلي والوريد الوداجي الداخلي بالموجات فوق الصوتية في توقع انخفاض ضغط الدم المطول مع التخدير الكلي ، وقد تم إجراء الدراسة في مستشفى القصر العيني بجامعة القاهرة على المرضى الخاضعين للعمليات الغير قلبية والغير طارئة تحت التخدير الكلي .فَحص مئة وثلاثة وستين (163) مريض لتحديد إمكانية إجراء الدراسة عليهم واستبعد منهم ستة (6) مرضى لعدم استيفاءهم شروط الدراسة كما استبعد أربعة عشر )14) مريض لصعوبة فحص الوريد الأجوف السفلي بالموجات فوق الصوتية وكذلك عشرة (10) مرضى بسبب طول مدة وضع الانبوبة الحنجرية ، وتم إجراء الدراسة على المئة وثلاثة وثلاثين (133) مريض المتبقين .أظهرت الدراسة أن معدل تغير الوريد الوداجي الداخلي بالموجات فوق الصوتية بمقدوره توقع انخفاض ضغط الدم مع التخدير الكلي وكذلك توقع الانخفاض الشديد لضغط الدم وأنه ذات قيمة تنبؤية متوسطة ؛ أما بالنسبة للوريد الأجوف السفلي فقد أظهرت الدراسة أنه لا يمكن لأي من قياساته توقع انخفاض ضغط الدم مع التخدير الكلي . |
