| Summary, etc. | Neuronal progranulin (PGRN) overexpression, is one of the endogenous adaptation processes of pain defense following nerve injury. It indorses the survival of injured neurons to hinder the enhanced nociceptive response. Trimetazidine (TMZ) has been widely utilized by cardiac patients as an anti-anginal drug due to its anti-ischemic property. It has been shown to promote axonal regeneration of sciatic nerves subjected to crush injury. The study aimed to explore PGRN and extracellular signal-regulated kinases (ERK1/2) interplay as a mechanistic tool through which TMZ could alleviate neuropathic pain following paclitaxel (PTX) administration. Rats were given four injections of PTX (2 mg/kg, i.p) every other day. Two days after the last dose, rats received TMZ (25mg/kg) with or without the ERKinhibitor, PD98059, for 21 days. TMZ maintained the integrity of myelinated nerve fibers, as reflected on the obvious reduction in the axonal damage biomarkers. Accordingly, it alleviated PTX-evoked thermal, cold and mechanical allodynia/hyperalgesia. TMZ promoted ERK1/2 phosphorylation with a profound upsurge in PGRN content. These effects were associated with a marked increase in Notch1 receptors gene expression and a prominent anti-inflammatory effect with a marked increase in mRNA expression of secretory leukocyte protease inhibitor. In addition, TMZ diminished oxidative stress as well as caspase-3 concentration in the sciatic nerve. On the contrary, co-administration of PD98059 completely reverted these beneficial effects. Taken together, the present study demonstrates the robust antinociceptive effect of TMZ that could be largely attributed to its boosting effect on PGRN and Notch1 receptors via ERK1/2 activation |

| Summary, etc. | يعتبرالتهاب الأعصاب الطرفية أحدأهم الآثار الجانبية الرئيسية لاستخدام عقار الباكليتاكسيل، وإلى الآن لم توافق إدارة الغذاء والدواء على استخدام أية أدوية للحد من هذا الأثر.بينما تعمل الأدوية التقليدية مثل مضادات الاكتئابومضادات الصرع والمسكنات الأفيونيةعلى تسكين الألم جزئيًا في 40% إلى 60% فقط من المرضى.تهدف الدراسة الحالية إلى اختبار التأثير الملطف للترايميتازيدين على التهاب الأعصاب الطرفية المحدث بواسطة الباكليتاكسيل في الجرذانبالإضافة إلى اكتشاف المسارات المحورية لهذه الآليةالمترتبة على استخدام الترايميتازيدين.تم توزيع الجرذان عشوائيا إلى أربع مجموعات حيث ضمت كل مجموعة تسعة جرذان. كانت المجموعة الأولى تمثل المجموعة الضابطة، بينما تم حقن المجموعة الثانية بدواء الباكليتاكسيل(2 مجم/كجم) في التجويف البريتوني لمدة 4 أيام متناوبة (0، 2، 4، 6). وأعطيتالمجموعتان الثالثة والرابعةدواء الترايميتازيدين عن طريق الحقن في التجويف البريتوني بجرعة (25مجم/كجم).في حين تلقت الجرذان في المجموعة الرابعة مثبط إرك بي دي (98059) بجرعة(0,3مجم/كجم) مذابًا في 0,1 % ثنائي ميثيل سلفوكسيد عن طريق الحقن في التجويف البريتوني وذلك قبل حقن الترايميتازيدين بثلاثين دقيقة.وجرى حقن كلا من مثبط إرك والترايميتازيدين يوميًا لمدة 21 يومًا بدءًا من اليوم الثامن من حقن الباكليتاكسيل. |
