| Summary, etc. | With better understanding of corneal biomechanics and reports of ectasia <br/>following Lasik procedures, the range of Myopic treatment has narrowed using any <br/>type of laser refractive surgery whether PRK, LASIK or SMILE. Safety is a major <br/>issue in any surgical procedure more so in refractive procedures. Posterior chamber <br/>Phakic intraocular lenses has been used for a very long time with high safety record. <br/>The ICL collamer material has shown excellent biocompatibility over the years and <br/>being in the posterior chamber away from the corneal endothelium adds to the safety <br/>profile. The incidence of cataract was in the range between 0.6-2.7 % in the model <br/>of v4, and there were some reports about incidence of long-term endothelial cell loss <br/>[1-3]. <br/>The Visian Implantable Collamer Lens V4c (ICL V4c) (ICL Model V4c; <br/>STAAR Surgical, Monrovia, CA, USA) is a new generation of posterior chamber <br/>phakic intraocular lens, which is capable to correct up to −18.00D of myopia [4]. <br/>Compared with the traditional ICL V4, the ICL V4c was designed with a <br/>central convex/concave optical zone diameter of 4.9–5.8 mm and a 0.36 mm central <br/>hole, which improves aqueous humor circulation and obviates the need for <br/>peripheral iridotomy or iridectomy |

| Summary, etc. | يشكل ضعف النظر مشكله عند اغلب المرضى, و تتعدد انواع ضعف النظر منها قصر النظر وطول النظر والاستيجماتيزم, وقد تعددت طرق علاج ضعف النظر بتقدم التاريخ والعلم من النظارات الطبية والعدسات اللاصقة والليزك (تصحيح الابصار بالليزك).<br/> ولكن ليس كل انواع ضعف النظر تٌعالج بتصحيح الابصار بالليزك لذا زرع العدسات بداخل العين سواء فى الخزانه الاماميه للعين والمثبتة فى زاويه العين او المثُبتة فى قزحية العين او تلك التى تٌزرع بالخزانه الخلفية للعين مثل عدسات الكوالمير المرنة امام العدسة البلورية للعين. <br/>ويُعد زرع العدسات داخل العين في وجود العدسة البلورية خاصة في حالة قصر النظر الشديد من الطرق الجيدة حيث أنها تحافظ على العدسة البلورية و بالتالي عدم فقد القدرة على الرؤية القريبة، بالإضافة إلى سهولة تبديل العدسة في حالة تغير النظر، وكذلك المحافظة على الشكل الطبيعي للقرنية وسمكها و بالتالي المحافظة على دقة حدة الإبصار.<br/>وعدسات الكوالمير المرنة يتم وضعها بين الجسم القزحي و العدسة البلورية فقد تؤدي إلى مياه بيضاء أو ارتفاع في ضغط العين، لكن التصميم الحديث منها يعد آمنا و ثابتا داخل العين. <br/>كما يتضمن التصميم الحديث منها على فتحة دائرية بمنتصف تلك العدسه ليضمن حريه حركه الخلط المائى وذلك تجنُبا لارتفاع ضغط العين بعد زراعه تلك العدسات و بالتالي لا يحتاج التصميم الحديث الي عمل فتحة بديلة في القزحية.<br/> تابعت هذه الدراسة المرضى الذين يعانون من ضعف حاد بالنظر و قاموا بزراعة عدسات الكوالمير المرنة ذات التصميم الحديث, و أظهرت هذه الدراسة أن العدسات المزروعة أدت إلى نتائج ممتازة من حيث ملائمتها لاصلاح قصر النظر الشديد مع امكانية ازالتها إذا أدت إلى مضاعفات شديدة، كذلك كفائتها في اصلاح حدة الإبصار مع جودة عالية في حدة الرؤية، و ذلك لتكبيرها حجم الصورة الواقعة على الشبكية بالإضافة إلى عدم تغييرشكل القرنية و قدرة العدسة البلورية على الرؤية القريبة. |
