| Summary, etc. | Malnutrition is a persistent problem in hospitals and 60% of<br/>the patients are either malnourished or are at nutrition.<br/>Objective: Assessment of the nutritional status & risk in patients with GI and<br/>liver diseases admitted to ICU using the most valid assessment tools and<br/>determine the reliability of these tools in identifying those patients with the<br/>possible impact on the clinical outcomes.<br/>Methods: A prospective observational study was performed on 150 patients<br/>with GI and liver diseases admitted in ICU for ≥ 48 hours, and underwent<br/>nutritional assessment using subjective global assessment (SGA), nutritional<br/>risks screening 2002 (NRS-2002), and modified NUTRIC score (mNUTRIC-<br/>score) followed by evaluation of the clinical outcomes.<br/>Results: Study revealed that 102 patients (68%) of the studied patients were<br/>malnourished, 86 patients (57.3%) were at medium to high risk, and the total<br/>mortality rate reached (23.3%). Severe malnourishment was associated with<br/>older age > 61 years (69.4%), hepatobiliary diseases and abdominal<br/>malignancies (57.1% and 34.7% respectively), normal weight BMI (57.1%),<br/>with high mean of CCI, SOFA, and APACHE II scores (8.2±3.9, 11±4.9, and<br/>27±7.3 respectively) which reflected higher mortality rate (51%), and longer<br/>length of stay (10±7.4 days). According to nutritional risk, the high-risk patients<br/>(n= 48, 32%) were also associated with older age > 61 years (81.3%), had<br/>hepatobiliary diseases and abdominal malignancies (47.9% and 50%<br/>respectively), reported normal weight BMI (58.3%), and high mean of CCI,<br/>SOFA, and APACHE II scores (9±3.3, 13±4, and 30±6.4 respectively) which<br/>reflected higher mortality rate (62.5%), and longer length of stay (13±6.2 days).<br/>Conclusion: The clinical outcomes could be affected by the nutritional status<br/>and risk as malnourished patients and those who were at high risk were<br/>associated with poor clinical outcomes in form of higher incidence of mortality<br/>and morbidities with longer length of stay (LOS). SGA, NRS-2002, and<br/>mNUTRIC-score are reliable tools in assessment of nutritional status & risk and<br/>have the ability to predict clinical outcomes |

| Summary, etc. | هذه الدراسة تعتبر دراسة استباقية قائمة على الملاحظة التي أْجريت على 150 مريضًا ممن يعانون من أمراض الجهاز الهضمي والكبد بالرعاية المركزة، كان الهدف منها هو تقييم الحالة الغذائية ومخاطرها وتأثيرها على النتائج السريرية باستخدام أدوات الفحص الطبي والتقييم الغذائي الإكلينيكي وبذلك تقييم مدى صحة وموثوقية هذه الأدوات في تحديد المرضى الذين من المرجح أن يستفيدوا من الدعم الغذائي المعوي مع التأثير المحتمل على النتائج السريرية.<br/>نتائج الدراسة: وُجد أن ١٠٢ مريضاً (٦٨٪) من المرضى الذين خضعوا للدراسة يعانون من سوء الحالة الغذائية وكان معدل الوفيات بينهم ٣٣ مريضاً (٣٢.٣٪)، بينما ٤٨ مريضاً (٣٢٪) من المرضى كانوا ذو حالة تغذوية جيدة وتُوفي منهم مريضان فقط (4.2٪). فيما يتعلق بالمخاطر الغذائية، وجد أن ٨٦ مريضاً (٥٧.٣٪) من الذين خضعوا للدراسة كانوا معرضين لخطورة متوسطة إلى عالية وكان معدل الوفيات بينهم ٣٣ مريضاً (٣٨.٣٪)، بينما ٦٤ مريضاً (٤٢.٧٪) كانوا ذو مخاطر غذائية منخفضة وتُوفي منهم مريضان فقط (٣.١٪).<br/>لوُحظ أيضًا أن المرضى الذين يعانون من سوء الحالة الغذائية والمرضى المعرضين لمخاطر غذائية عالية كانوا مرتبطين بمدة إقامة أطول وانتشار أعلى للأمراض بالرعاية المركزة مقارنة بالمرضى ذوي الحالة الغذائية الجيدة وذوي المخاطر الغذائية المنخفضة.<br/>استخلصت الدراسة إلى أن النتائج السريرية للمرضى الذين يعانون من أمراض الجهاز الهضمي والكبد بالرعاية المركزة تتأثر بشكل واضح بحالتهم الغذائية ومخاطرها إلى جانب الدعم الغذائي المعوي المبكر. اقترحت الدراسة أيضًا أن أدوات الفحص الطبي والتقييم الغذائي الإكلينيكي مثل: - (SGA)، (NRS-2002 score)، و (score mNUTRIC) هي أدوات ذات صحة وموثوقية في تقييم الحالة الغذائية ومخاطرها، ولديها القدرة على التنبؤ بالنتائج السريرية بشكل مستقل عن حدة المرض ولتحديد المرضى الذين من المرجح أن يستفيدوا من الدعم الغذائي.<br/>تُوصي الدراسة بأن يتم خضوع جميع المرضى ممن يعانون من أمراض الجهاز الهضمي والكبد بالرعاية المركزة لتقييم الحالة الغذائية ومخاطرها في غضون ٢٤-٤٨ ساعة الأولى لتشخيص سوء التغذية وتقييم مخاطر الوفيات.<br/> |
