| Summary, etc. | Introduction: Children with congenital heart diseases (CHD) undergoing open heart surgeries for repair of cardiac defects are at risk for complications of cardio-pulmonary bypass machine (CPB). <br/>Objectives: - This study is designed to detect the effect of CPB on thyroid function test in Children with CHD undergoing open heart surgeries for repair of their cardiac defects.<br/> Patients and Methods: - This is a Cross sectional analytic study that includes 50 patients with congenital heart defects, 25 of them were cyanotic and 25 of them were acyanotic. This study was performed during the period from May 2022 to April 2023 in patients undergoing cardiac surgery for repair by assessment of thyroid function tests pre and post CPB.<br/>Results: There was high significant statistical difference between cyanotic and acyanotic groups regarding BYPASS Time, X-Clamp, ICU stay / day, Duration of MV / day and inotropic score, There was no significant statistical difference found between cyanotic and acyanotic groups regarding preoperative TSH levels while there was a highly significant statistical difference between them regarding fT4 (preop) and significant statistical difference between them regarding fT3 (preop), There was significant statistical difference between cyanotic and acyanotic groups regarding postoperative TSH levels while there was no significant statistical difference found between them regarding postoperative fT4 and fT3, There was significant statistical positive correlation between fT3 (preop) and So2 (preop), There was high significant statistical Negative correlation between fT4 (preop) with So2 (preop), There is high significant statistical Negative correlation between fT3 (preop) with bypass time, x-clamp, duration of MV, ICU stay and inotropic score, There was high significant statistical positive correlation between fT4 (preop) with bypass time, x-clamp, duration of MV, ICU stay and inotropic score, There was high significant statistical positive correlation between TSH (postop) with so2 (preop), There was significant statistical Negative correlation between TSH (postop) with bypass time, x-clamp, duration of MV, ICU stay and inotropic score, There was significant statistical negative correlation between fT3 (postop) with duration of MV, ICU stay and inotropic score, There was significant statistical correlation between fT3 (preop) with Mortality post open heart surgery, There was a high significant statistical difference between TSH (preop) and TSH (postop) and between fT3 (preop) and fT3(postop).<br/>Conclusion: - The present study showed that CPB was affecting thyroid function so possibly affecting the results of open-heart surgeries and ICU stay and duration of MV. |

| Summary, etc. | انه من المهم جدا متابعه هرمونات الغده الدرقيه قبل و بعد جراحات القلب المفتوح في الاطفال اللذين يعانون من عيوب خلقيه بالقلب و يخضعون لجراحات قلب مفتوح لاصلاح هذه العيوب الخلقيه باستخدام ماكينه القلب الصناعي.<br/>وبما ان عميات القلب المفتوح لاصلاح العيوب الخلقيه بقلب الاطفال تسبب اضطراب في وظائف الغده الدرقيه و ما ينتج عنه من اثار و مضاعفات وتأثير علي أجهزه الجسم المختلفه لذا قد كان لزاما اجراء هذه الدراسه حيث ان هذه الدراسه تهدف الي توضيح العلاقه بين تأثير ماكينه القلب الصناعي علي هرمونات الغده الدرقيه و كذلك نتائج جراحات القلب المفتوح للاطفال الذين يعانون من عيوب خلقيه بالقلب بعد اجراء جراحه قلب لاصلاح العيوب الخلقيه.<br/> لقد اشتملت هذه الدراسه علي عدد 50 طفل من الاطفال الذين يعانون من عيوب خلقيه بالقلب منهم 25 طفل يعانون من عيوب خلقيه بالقلب تسبب الزرقه و 25 طفل منهم يعانون من عيوب خلقيه بالقلب لا تسبب الزرقه. وقد تم اجراء هذه الدراسة في الفتره من مايو 2022 و حتي أبريل 2023 علي الاطفال الذين خضعوا لاجراء جراحه قلب مفتوح لاصلاح العيوب الخلقيه حيث قد تم قياس هرمونات الغده الدرقيه قبل و بعد جراحات القلب المفتوح للاطفال.<br/>وقد اوضحت الدراسه ان متابعه هرمونات الغده الدرقيه في الاطفال الذين يعانون من عيوب خلقيه في عضله القلب قبل وبعد عمليات القلب له دور في تحسين نتائج جراحات القلب المفتوح للاطفال حيث انه يعتبر مؤشر مهم لنتائج جراحات القلب المفتوح و يؤثر علي فتره الاقامه بالرعايه المركزه و كذلك فتره جهاز التنفس الصناعي و ايضا يساعد علي تجنب المضاعفات المحتمل حدوثها.<br/>كما اوضحت الدراسه ان هناك علاقه احصائيه مهمه جدا بين هرمون fT3 و حدوث الوفاه بعد جراحات القلب المفتوح للاطفال.<br/>كما اوضحت الدراسه ان هناك فرق احصائي كبير بين الاطفال اللذين يعانون من عيوب خلقيه بالقلب تسبب الزرقه و اللذين يعانون من عيوب خلقيه بالقلب لا تسبب الزرقه من حيث توقيت ماكينه القلب الصناعي و كذلك توقيت الجهاز التنفس الصناعي و فتره الاقامه في الرعايه المركزه.<br/>كما اوضحت الدراسة ان هناك فرق كبير بين نتائج هرمونات الغده الدرقيه TSH, fT3 قبل وبعد ماكينه القلب الصناعي و اللتي تعكس بشكل مباشر تأثير ماكينه القلب الصناعي علي هرمونات الغده الدرقيه و اللتي يؤثر بدورها علي اجهزه الجسم المختلفه |
