Summary, etc. Background: Usually, a multimodality approach is employed in treating breast cancer in the form of systemic treatment, surgery and radiotherapy. Postoperative radiation therapy can have a detrimental effect on the breast due to local complications. In certain types of tumors, neoadjuvant radiotherapy is the standard of care. Recently, there is a trend towards utilizing such approach in breast cancer. The rationale behind neoadjuvant concurrent chemoradiation in breast cancer is that the same sequence of treatment showed excellent results in other tumor sites with improved local control and survival benefits and shorter treatment duration.<br/>Objective: To assess the surgical outcome after using concurrent chemoradiation in the neoadjuvant setting in breast cancer in terms of wound healing and postoperative complications (e.g.: seroma, infection).<br/>Methods: This is a prospective single-arm study, where eligible patients will receive neoadjuvant concurrent chemoradiation followed by surgery, and then patients will be followed-up for at least 2 months to evaluate the surgical outcome. The primary endpoint will be the rate of postoperative complications, especially skin necrosis, while the secondary endpoint will be the rate of pathological complete response (pCR) in tumors.<br/>Results: A total of 80 patients were included in the study. The total rate of postoperative complications was 10% (n=8). Only 5 patients suffered from postoperative surgical site infection, and only 3 patients (3.75%) had postoperative skin necrosis. All patients healed within 2 months postoperatively.<br/>Conclusion: This study showed that preoperative radiation therapy given concurrently with neoadjuvant systemic therapy is a safe and feasible technique with no increase in postoperative morbidity with different types of surgery and it streamlines the treatment process.

Summary, etc. في الآونة الأخيرة، تم استخدام العلاج الجهازي بشكل متزايد في بيئة المواد المساعدة الجديدة لسرطان الثدي وقد ثبت أن فوائد العلاج المساعد الجديد والعلاج المساعد متقاربة، مع وجود فائدة إضافية للعلاج المساعد الجديد تتمثل في زيادة الاستجابة المرضية التي تعد بديلاً لتحسين البقاء على قيد الحياة.<br/>العلاج الإشعاعي المساعد الجديد هو معيار الرعاية في بعض الأورام (مثل المريء والمستقيم) ويتم تقديمه بالتتابع مع العلاج الكيميائي المساعد الجديد كمحسس إشعاعي، مما يؤدي إلى تحسين الاستجابة المرضية وانخفاض معدلات النقائل البعيدة.<br/>وقد ثبت وجود معدل أعلى من الاستجابة الكاملة للعلاج في مرضى سرطان الثدي الذين عولجوا بالعلاج الكيميائي والإشعاعي المتزامن المساعد الجديد، ولكن لا تزال هناك شكوك حول جدوى وسلامة هذه التقنية فيما يتعلق بمعدل معدل مضاعفات ما بعد الجراحة، وخاصة نخر الجلد، بسبب التأثير السلبي للعلاج الإشعاعي على حيوية الجلد.<br/>أظهرت هذه الدراسة أن معدل مضاعفات ما بعد الجراحة بعد إعطاء العلاج الكيميائي والإشعاعي المتزامن ما قبل الجراحة لسرطان الثدي يقع ضمن الأرقام المقبولة الواردة في الأدبيات، وأنه ساعد في تبسيط جدول العلاج وتقليل الانقطاعات وعدم الامتثال، ومن ثم فهو آمن للاستخدام في مجموعة مختارة من المرضى الذين يخضعون للعلاج من سرطان الثدي.
