Summary, etc. Aim: To assess the diagnostic accuracy of cone-beam computed tomography (CBCT) dose reduction on the mandibular canal visibility compared to standard CBCT dose, in cases requiring assessment of mandibular canal to plan conservative or radical treatment in the mandibular posterior region.<br/>Methodology: Ten human dry mandibles were scanned six times using a Planmeca ProMax® 3D Mid CBCT machine, employing three conventional CBCT protocols and three ultra-low CBCT protocols. Varying combinations of milliampere-second (mAs) and voxel size values were utilized across these protocols. The clarity of mandibular canal visibility was evaluated using a two-scale scoring system, comparing an ultra-low dose protocol (42.6 mAs/0.2 mm) to the gold standard protocol (180 mAs/0.2 mm). Additionally, the degree of mandibular canal visibility was assessed using a five-scale scoring system, comparing three ultra-low dose protocols (42.6 mAs/0.2 mm, 25.2 mAs/0.4 mm, and 9.9 mAs/0.6 mm) and two conventional protocols (108 mAs/0.4 mm and 50.4 mAs/0.6 mm) to the gold standard protocol. Gutta-percha points were used as reference markers to standardize the area under investigation. Comparative analysis was conducted on 2-dimensional trans-axial images by two observers. Results: The results showed that an ultra-low dose protocol (42.6 mAs/0.2 mm) achieved similar clarity to the standard dose (180 mAs/0.2 mm) when using a simple two-point scoring systemHowever, a more detailed five-point scale revealed significant differences, with higher milliampere-seconds (mAs) and smaller voxel sizes leading to better visibility scores. Importantly, inter-observer agreement remained good for both scoring methods.<br/>Conclusion: Ultra-low dose CBCT appears to be a viable option for tasks requiring clear visualization of the mandibular canal. While standard dose protocol might be preferable for situations demanding more precise canal definition, this study suggests the potential for CBCT dose reduction in clinical applications requiring MC visualization, potentially enabling more conservative treatment approaches.
