Summary, etc.	Low back pain is the leading cause of years lived with disability globally. In 2018, an international working group called on the World Health Organization to increase attention on the burden of low back pain and the need to avoid excessively medical solutions.<br/>Indeed, major international clinical guidelines now recognize that many people with low back pain require little or no formal treatment. Where treatment is required the recommended approach is to discourage use of pain medication, steroid injections and spinal surgery, and instead promote physical and psychological therapies. Many health systems are not designed to support this approach.<br/>The key challenges of low back pain care within health systems include the financial interests of pharmaceutical and other companies; outdated payment systems that favour medical care over patients’ self-management; and deep-rooted medical traditions and beliefs about care for back pain among physicians and the public.

Summary, etc.	آلام أسفل الظهر (الام اسفل الظهر) هي سبب رئيسي للإعاقة الكبيرة ولها انتشار شبه مستوطن في المجتمعات الغربية. تكاليف الإنتاجية المفقودة من أيام العمل الضائعة والنفقات المتعلقة بمعالجه الام اسفل الظهر مذهلة.<br/>الحلقات "الحادة" من الام اسفل الظهر، والتي تمثل ما يصل إلى 90٪ من الحالات ، يتم حلها دون علاج جائر في غضون عدة أسابيع.<br/>يشار إلى الام اسفل الظهرعلى أنه "مستمر" عندما تستمر الأعراض لفترة أطول من 4 إلى 6 أسابيع وعلى أنها "مزمنة" عندما يستمر الألم بعد 12 أسبوعًا. عندما تكون جذور الأعصاب القطنية العجزية متورطة ، فإن أعراض اعتلال الجذور أو الألم الجذري المصاحب يمكن أن تكون سائدة. تشمل عوامل الخطر لتطور واستمرار الام اسفل الظهرالمزمن العوامل التشريحية والفسيولوجية والنفسية الاجتماعية والسلوكية والمالية . نظرًا لأن احتمالية الشفاء تتناقص مع مدة الام اسفل الظهر، فإن هدف الطبيب المسؤول هو إعادة مرضى الام اسفل الظهرإلى مستويات النشاط الطبيعي بالسرعة التي تسمح بها شدة أعراضهم. في حالة عدم وجود اي حالة خطيرة مثل العدوى أو الورم ، يتم توجيه التدخلات الفعالة نحو تقليل مستويات الألم ووقت التعافي بالإضافة إلى تحسين الوظائف. عادة ما يكون علاج الام اسفل الظهرالمزمن مع أو بدون أعراض جذرية أكثر فعالية في بيئة تعالج تعقيدها وتشمل 1) العلاج الطبيعي وإعادة التأهيل) التدخلات النفسية مثل العلاج المعرفي السلوكي. العلاج الدوائي. وإجراءات الألم التداخلية مثل (حقن الستيرويد فوق الجافية العلاجي - التحلل فوق الجافية للالتصاقات - تحفيز الحبل الشوكي - أجهزة التسريب داخل القراب - رأب العمود الفقري - تقويم الحدبة - كتل الفروع الإنسي والاستئصال بالتردد الحراري).<br/>الهدف من الدراسة:<br/>مناقشة دور إجراءات الألم التدخلية ودليل على فعاليتها في علاج آلام أسفل الظهر المزمنة.
