Summary, etc.	Two equal groups of forty stroke patients of both sexes were randomly allocated. For eight weeks, the study group (group A) received three sessions per week of combined treatment consisting of systematic desensitization for 50 minutes and a goal-directed paradigm for 40 minutes, while the control group (group B) received just the goal-directed paradigm for 40 minutes. The timed up and go test (TUG) and Dynamic Gait Index (DGI) were used to assess function performance; the Berg Balance Scale (BBS) and Biodex fall risk index (FRI) were used to evaluate balance; and the 16-item Fall Efficacy Scale - International (FES-I) was used to evaluate fear of falling. At baseline and following treatment, all measurements were obtained. Systematic desensitization combined with a goal-directed paradigm has a superior effect on improving functional performance, reducing the risk of falling and the fear of falling in patients with stroke in comparison to a goal-directed paradigm only.<br/><br/>

Summary, etc.	تم إختيار 40 مريضا من مرضي السكتة الدماغية المزمنة من الجنسين ، يعانون من خطر السقوط . تراوحت أعمارالمرضي من 45 الى ٦٠ عاما. تم توزيع المشاركين إلي مجموعتين متساويتين بشكل عشوائي. تلقت مجموعة الدراسة (المجموعة أ) ثلاث جلسات أسبوعيًا من العلاج المشترك المكون من إزالة الحساسية المنهجية لمدة 50 دقيقة والنموذج الموجه للهدف لمدة 40 دقيقة، في حين تلقت المجموعة الضابطة (المجموعة ب) نفس النموذج الموجه للهدف فقط لمدة 40 دقيقة، وذلك لمدة ثمانية أسابيع. تم تقييم مرضي المجموعتين قبل وبعد برنامج العلاج بإستخدام نظام توازن بيوديكس لتقييم خطر السقوط، ومقياس توازن بيرج ، ومقياس فعالية السقوط الدولي (I) المكون من 16 عنصرًا ، ومؤشر المشية الديناميكي وإختبار التوقيت والذهاب وان إزالة الحساسية المنهجية المقترن بالنموذج الموجه للهدف له تأثيرفعال ومتفوق على تحسين الأداء الوظيفي، وتقليل خطر السقوط والخوف من السقوط لدى مرضى السكتة الدماغية المزمنة مقارنة بالنموذج الموجه للهدف فقط.
