Summary, etc.	Purpose: Assessment of feasibility of combined vascular resection with pancreaticoduodenectomy for borderline resectable pancreatic head cancer in National Cancer Institute as well as assessment of associated postoperative morbidity and mortality of combined vascular resection with pancreaticoduodenectomy.<br/>Methods: 20 patients diagnosed with borderline resectable pancreatic cancer underwent pancreaticoduodenectomy with vascular resection at the national cancer institute. Preoperatively, upon patients admission into the surgical ward, full demographic data was documented for all patients as well as the co-morbidites and the risk factors for pancreatic cancer. Intraoperatively, the variable surgical techniques of the vascular resection were described with intra-operative illustrative pictures. Operative time was recorded, as well as estimated blood loss. Postoperatively, patients were clinically assessed whether at the ICU,surgical ward and at the out-patient clinic after discharge for the reporting of the 30-day post-operative morbidity and the overall 30-day post-operative mortality.<br/> Results: Eight patients had partial venous excision with direct closure (venorraphy) by suture closure (type 1 PV resection), another eight patients had partial venous excision and vascular reconstruction using a falciform patch (type 2 PV resection). Four patients had segmental resection with primary veno-venous anastomosis (type 3 PV resection). Two patients (10%), one with type 2 and another patient with type 3 PV resection, had diminished liver portal flow on intraoperative duplex for which internal jugular vein grafting was done. There was no hospital mortality, with one 30 day-mortality case occurring on postoperative day 22 There was no hospital mortality, with one 30 day-mortality case occurring on postoperative day 22. Most common postoperative complication was delayed gastric emptying, occurring in six patients.Five patients had pancreatic fistula, all of which were grade A fistula with no therapeutic interventions and spontaneous resolution.<br/>Conclusion: although technically challenging, combined vascular resection with pancreaticoduodenectomy is a feasible surgical option for borderline resectable pancreatic head cancer.

Summary, etc.	يعتبر سرطان البنكرياس من بين السرطانات الأكثر شيوعاً حول العالم حيث تم إكتشاف حالات مايقرب من نصف مليون حالة جديدة خلال عام ٢٠١٨ المنصرم، ولكن من ناحية أخري يعتبر من أقوي السرطانات فتكاً بالإنسان حيث يقتل حوالي ثلث مليون مريض كل عام تبعاً للإحصاءت العالمية، ويمثل سرطان رأس البنكرياس حوالي ثُلُثي حالات سرطان البنكرياس كلها.<br/>وعلي النقيض من سرطانات البنكرياس الأخرى والتي تكتشف عادةً في مرحلة متأخرة، فإن حالات سرطان رأس البنكرياس تمثل تحدي للعلاج الجراحي لإكتشافها في مرحلة مبكرة من المرض حيث تكون واضحة الحدود وبحجم صغير وبدون إنتشار واسع.<br/>ولكن التحدي الأكبر للجراح في في علاج مرض سرطان رأس البنكرياس المكتشف مبكراً هو أن الأمل الوحيد لشفاء هؤلاء المرضي هو الإستئصال الجراحي وبعكس المتوقع فإن النتائج العالمية الحديثة لمآل المرضي بعد إجراء الجراحة تظهر أن حوالي ٦٪ فقط من المرضي ظلوا علي قيد الحياة بعد خمس سنوات من الجراحة وقد تتحسن النتائج إذا كان الورم أقل من ٢سم وفي مرحلة "تي ١" أما الحالات التي لم يتمكن الجراح من الإستئصال الكامل للورم أثناء الجراحة فيتساوي مآل العلاج الجراحي مع عدمه.<br/>ويعد السبب الرئيسي لعدم قدرة الجراح علي الإستئصال الكامل للورم بالرغم من صغر حجمه وعدم إنتشاره هو وجود أوعية دموية ملاصقة للورم مما قد يؤدي إلي عدم ترك حدود آمنة من الخلايا الغير سرطانية في نهاية العملية الجراحية.<br/>وسوف يقوم البحث بتقييم إمكانية إضافة خطوة الإستئصال الجراحي للأوعية الدموية الملاصقة لسرطان رأس البنكرياس أثناء عملية الاستئصال الجراحي للبنكرياس والإثني عشر بين مرضي معهد الأورام القومي-جامعة القاهرة المصابين بسرطان رأس البنكرياس والممكن علاجهم جراحياً ويتميزون بصفات “BORDERLINE-RESECTABLE “
