| Summary, etc. | Open surgery remains one of the approaches used for those patients requiring partial or radical nephrectomy and is associated with a high incidence of intense immediate postoperative pain and chronic pain the months following surgery. Adequate perioperative pain control is an essential requirement for pediatric anesthesia, because Improper pain control can result in deleterious postoperative morbidities. <br/>Central nerve blocks are often used in combination with general anesthesia for pediatric surgery to reduce general anesthetic requirements, opioid use, postoperative pain, nausea and vomiting, and risk of anesthetic neurotoxicity, particularly in young patients.<br/>Caudal epidural analgesia is one of the commonly used blocks in different pediatric surgical interventions. However, it is more commonly used for surgeries below the umbilicus, it became a well-established and commonly performed neuraxial technique for providing intraoperative and postoperative analgesia in pediatric patients scheduled for lower abdominal surgical interventions.<br/>Although the efficacy and safety of CEB are high, the associated complications such as inadvertent dural puncture, unwarranted motor blockade of lower limbs, and disturbance of bladder function might limit its use.<br/>The ultrasound‐guided erector spinae plane block is a novel regional anesthetic technique that was first described for the management of acute and chronic thoracic pain. It is an easy, safe, and effective fascial plane block technique used for different pediatric surgical procedures. It involves injection of local anesthetics into the fascial plane situated deep to the erector spinae muscle with subsequent craniocaudal spread of injectate. Local anesthetics also penetrate anteriorly to enter the thoracic paravertebral space where it blocks both the ventral and dorsal rami of spinal nerves as well as the rami communicates that convey sympathetic fibers. Taking into consideration the extension of the erector spinae muscle and erector spinae plane down to the lumbar spine, the erector spinae plane block is assumed to offer visceral and somatic abdominal analgesia if the injection was conducted at a lower thoracic level.<br/>The study aimed to compare the perioperative analgesic effect of ultrasound-guided erector spinae plane block and ultrasound-guided caudal block in pediatric cancer patients undergoing nephrectomy surgeries.<br/>This prospective, randomized controlled trial was performed on 77 pediatric patients who underwent nephrectomy surgery. Patients allocated into two groups: Group C (n = 39) ultrasound guided caudal epidural block (CEB) received 1.25 ml/kg of 0.125% bupivacaine. Group E (n= 38) ultrasound guided erector spinae plane block (ESB) received 0.5mL/kg of 0.25% bupivacaine. |

| Summary, etc. | تظل الجراحات لاستئصال اورام الكلي احدي طرق علاج اورام الكلي عند الاطفال و ترتبط بارتفاع معدل حدوث الم شديد بعد العملية الجراحية و قد يؤدي الي حدوث الم مزمن في الاشهر التالية.<br/>و لذلك من الضروري معالجة الم ما بعد الجراحة عند الاطفال حتي نتجنب حدوث مضاعفات .<br/>يوجد الكثير من الاساليب المتبعة لعلاج الالم اثناء و بعد الجراحات عند الاطفال مثل : حقن الادوية الوريدية كالمواد الافيونية و الادوية المضادة للالتهاب غير الستيرويدية , والتخدير الموضعي في الجرح وتقنيات التخدير الموضعية مثل التسكين فوق الام الجافية في المنطقة الذيلية، والمنطقة المجاورة للفقرات كتسكين المستوي اسفل العضلة الناصبة للفقرات في الفقرات الصدرية العليا.<br/>اسخدام الموجات فوق الصوتية جعل ممارسة التخدير أسهل في التصور وتحديد الوضع المعتاد و غير المعتاد للأعصاب والأوعية الدموية والإبرة أثناء مرورها عبر الأنسجة، وكذلك ترسيب وانتشار التخدير الموضعي في المستوى المطلوب وحول العصب المطلوب .<br/>تعتبر السدة فوق الام الجافية في المنطقة الذيلية واحدة من اشهر الطرق المتبعة لعلاج الالم اثناء و بعد الجراحات عند الاطفال , و لكن لا يخلو من امكانية حدوث بعض المضاعفات الجانبية مثل ثقب الم الجافية غير المقصود , حدوث ضعف الحركة في الساقين و اضطراب وظائف المثانة البولية كاحتباس البول .<br/>يعتبرالحقن أسفل العضلة الناصبة للفقرات حقن جديد نسبيًا وقد تم وصفه لأول مرة لعلاج آلام الاعتلال العصبي الصدري المزمن في عام 2016. وهو حقن المخدر في المستوى تحت العضلة، ولكن يمكن تصنيفه على أنه حقن محيط بالنخاع نظرًا لآلية عمله. وقد تم استخدامه في مجموعة متنوعة من الاستخدامات، كما هو الحال في آلام الأعصاب الصدرية وفي تسكين الألم بعد العملية الجراحية بعد جراحة البطن المفتوحة الكبرى. |
