| Summary, etc. | This study aims to assess the local control and Adverse Radiation Events (ARE) of staged volume GKRS for large skull based meningioma.<br/>Patients and methods<br/>This is a single arm hybrib study including 2 groups of patients (one group data was retrospectively reviewed and another group was recruited prospectively). This study was conducted at the Gamma Knife center (GKC), Nasser Institute Hospitals, Cairo. In the period from February 2010 to February 2021, 245 patients with large skull base meningiomas ≥ 10cc who underwent staged volume gamma knife treatment were included.<br/> Results:<br/><br/>• The median age of the patients was 49years (range, 14-77). One hundred and ninety two (78%) patients were females while fifty three (22%) patients were males. <br/>•The median treated total tumor volume was 23.2 cc (range, 8.4-82 cc). <br/>•Thirty one patients (13%) were staged over three treatment sessions, while 214 patients (87%) were treated in two treatment sessions <br/>•After a median follow up period of 17 months (range, 6-105 months), 117 patients (48%) had stationary course of the clinical symptoms, 102 patients (42%) had regressive course and 26 patients (7%) had progression of clinical manifestation either aggrevation of pre-existing symptoms or development of new ones.<br/>•Radiologically; 239 patients (97.9%) had a favorable radiological response including 124 patients (51%) had stationary disease and 115 patients (47%) had regressive disease while 5 patients (2%) had radiological disease progression and one patient with subjective assessment only during his visit. The median time for change in images was 12 months (range;6-99 months).<br/>•Post-GK visual field assessment included 201 patients; 146 patients (73%) had stable visual field and 42 patients (20%) had improved visual field, while 13 patients (7%) had worse visual field outcome.<br/>•Post-GK hearing assessment was done for 110 patients after treatment and compared to initial evaluation, of them 108 patients (98%) had stationary level of hearing ability and 2 patients (2%) had worse hearing outcomes after treatment |

| Summary, etc. | هذه دراسه لنتائج الجراحه الاشعاعيه المجزئه باستخدام مشرط جاما لعلاج الاحجام الكبيره من اورام الاغشيه السحائيه الحميده في قاع الجمجمه. أجريت هذه الدراسة في مركز جاما نايف، مستشفيات معهد ناصر، القاهرة. في الفترة من فبراير 2010 إلى فبراير 2021، تضمنت 245 مريضًا يعانون من أورام سحائية كبيرة في قاعدة الجمجمة بحجم ≥ 10 سم مكعب <br/>كان متوسط عمر المرضى 49 عامًا. كان مائة واثنان وتسعون (78٪) من المرضى من الإناث بينما كان ثلاثة وخمسون (22٪) من الذكورو كان متوسط حجم الورم الإجمالي المعالج 23.2 سم مكعب. تم تجزئة واحد وثلاثين مريضا (13٪) على مدى ثلاث جلسات علاجية، في حين تم علاج 214 مريضا (87٪) في جلستين علاجيتين.<br/>بعد فترة متابعة متوسطة تبلغ 46 شهرًا (المدى من 14 إلى 157 أشهر)، كان لدى 119 مريضًا (49%) ثبات في الاعراض الاكلينيكيه، وكان لدى 101 مريضًا (41%) تحسن في الاعراض الاكلينيكيه و25 مريضًا (10%) لديهم إما تفاقم الأعراض الموجودة مسبقا أو ظهور أعراض جديدة. <br/>أما عن متابعة المرض بفحوصات الرنين المغناطيسي علي المخ ل240 مريضًا (98.4٪) اظهروا استجابة للعلاج، بما في ذلك 97 مريضًا (40٪) لديهم حجم ثابت للورم و143 مريضًا (58.4٪) لديهم تحسن في حجم الورمبينما كان 4 مرضى (1.6٪) لديهم زيادة في حجم الورم .كان متوسط الوقت للتغيير في الرنين 18.8 شهرًا (المدى: .66-111.4 شهرًا).<br/>شمل التقييم البصري بعد العلاج بالجراحة الاشعاعية المجزئة 201مريضًا؛ كان لدى 144 مريضًا (72٪) مجالًا بصريًا مستقرًا و44 مريضًا (22٪) اظهروا تحسنا في مجال بصري ، بينما كان لدى 13 مريضًا (7٪) نتائج مجال بصري أسوأ. من بين 13 مريضًا يعانون من تدهور المجال البصري بعد العلاج؛ 3 مرضى اظهروا زيادة في حجم الورم علي الرنين المغناطيسي؛ 4 مرضى اظهروا في زيادة لحجم الارتشاح حول الورم؛ كان لدى 3 مرضى حجم الورم ≥20 سم مكعب؛ كان لدى مريض واحد عيب أولي شديد في المجال البصري للعين اليمنى العمياء مع تاثر شديد في مجال العين اليسرى وكان المريضان الاخريان يعانيان من تدهور طفيف في علاج المجال البصري دون تفسير واضح. وكان متوسط مدة التغيير في المجال البصري 16.7 أشهر (المدى، 8.2-88 شهرا) من العلاج. |
