| Summary, etc. | Background: Bennett`s fracture is an intra-articular fracture dislocation at the carpometacarpal joint of the thumb. The thumb metacarpal subluxates or dislocates away from the ulnar fragment. It is an unstable fracture, with inadequate treatment will lead to weakness, osteoarthritis, and loss of function of the thumb. The treatment of this fracture includes conservative with Spica cast, closed reduction with percutaneous k-wiring fixation, and open reduction internal fixation. The study aimed to evaluate the functional and radiological outcomes of surgical management of Bennett`s fracture.<br/>Patients and methods: A prospective study was conducted on twenty patients with Bennett's fracture between January 2022 and September 2023. Those who matched the inclusion criteria were included and followed up for an average of six months.Results: most patients (90%) had a satisfactory hand function with a mean quick dash score of 8.1 after closed reduction and fixation by k-wires. All patients with excellent functional results were noticed to have excellent radiological results.Conclusion: Our satisfactory results suggest that closed reduction and percutaneous pinning is an excellent method for treating Bennett`s fractures. |

| Summary, etc. | كسر بينيت هو كسر مفصلى فى قاعدة المشطية الاولى وهو كسر غير مستقر.طريقه العلاج غير المناسبة ممكن ان تؤدى الى خشونة فى المفصل او ضعف وعدم القدرة على حركة مفصل الابهام .تعتبر طرييقة رد الكسر ذات سهولة نسبية ولكن تكمن الصعوبة فى طريقة المحافظة على الكسر بعد ردة .<br/> تم عمل هذه الدراسة لتقييم النتائج الإشعاعية والوظيفية للعلاج الجراحى لكسر بينت. عشرون مريضا بكسر بينيت تم علاجهم بطريقة الرد المغلق والتثبيت باسلاك معدنية وتم متابعتهم لمدة حوالى ستة اشهر .كان متوسط عمر المرضى حوالى ستة وثلاثون عاما وتراوحت اعمارهم مابين تسعة عشر عاما واربعة وخمسون عاما وكانوا معظمهم رجالا باستثناء ثلاثة اناث .عند اختيار الحالات تم استبعاد من هم يعانون من كسر مفتوح او كسر مهمل او المرضى الغير مؤهلين صحيا لاجراء الجراحة او المرضى ذوات الكسور المفتتة (كسر رولاندو).<br/>وقد اشتمل هذا البحث على جزيئين <br/>الجزء الاول وهو التشريح الوظيفى لمفصل الابهام وطريقة حدوث كسر بينيت وكذلك طرق العلاج والتثبيت للكسر <br/>الجزء الثانى وهو الجزء العملى لهذا البحث واشتمل على <br/>1-الطريقة التى اتبعت لتثبيت الكسر عن طريق الرد المغلق والتثبيت بواسطة اسلاك معدنية و متابعة عشرون مريضا مدة على الاقل ستة اشهر ثم تقيم الحالات وظيفيا واشعاعيا .<br/>2-نتائج العلاج<br/>وفقاً لطرق التقييم السابقة، تم تصنيف النتائج الوظيفية على أنها ممتازة لـ 14 مريضاً (70%)، وجيدة لأربعة مرضى (20%)، ومقبولة لمريضين (10%)، وبدون نتائج سيئة. واعتبرت النتائج الممتازة والجيدة مرضية، في حين شملت النتائج غير المرضية النتائج المقبولة ؛ وبالتالي، فان النتائج المرضية كانت (90%) فى 18 مريضا والغير مرضية (10%) فى مريضين. <br/>كان هناك ارتباط كبير بين النتائج الإشعاعية والوظيفية. وقد لوحظ أن جميع المرضى الذين حصلوا على نتائج وظيفية ممتازة حصلوا على نتائج إشعاعية ممتازه . ايضا معظم المرضى الذين لديهم نتائج وظيفية جيدة (75%) حصلوا على نتائج إشعاعية جيدة. مريض واحد ذو نتائج وظيفية جيدة (25٪) حصل على نتائج إشعاعية ممتازة. مريضان مع نتائج مقبولة . أحدهما (50%) حصل على نتائج إشعاعية جيدة، والآخر (50%) حصل على نتائج إشعاعية مقبولة. ومع ذلك فان هناك حاجة إلى مدة أطول من المتابعة للحكم على تأثير فقدان الرد المفصلي وخشونة ما بعد الكسور على وظيفة اليد.<br/>فى هذة الدراسة حدثت بعض المضاعفات منها حدوث عدوى حول اسلاك التثبيت وتم علاجها عن طريق رفع الاسلاك واعطاء المضادات الحيوية المناسبة . كما حدث تيبس جزئى فى مفصل ابهام احد المرضى وتحسن بعد ممارسة العلاج الطبيعى<br/>فى نهاية الدراسة خلصنا الى ان كسر بينيت يمكن علاجة بطريقة فعالة عن طريق الرد المغلق والتثبيت بواسطة اسلاك معدنية |
