| Summary, etc. | Introduction: Hypertrophic scars (HSs) are caused by excessive collagen synthesis due <br/>to fibroblast proliferation. Botulinum toxin (BONT-A) slows fibroblast proliferation <br/>and collagen deposition, thereby improving HSs. Methods: This study enrolled 76 male <br/>and female patients aged 20 to 40 years with post-burn HS (PBHS), randomly allocated <br/>into experimental and control group followed a three-month program. The experimental <br/>group received BONT-A iontophoresis and traditional physical therapy (stretching <br/>exercises and scar massage), while the control group received only TPT program. The <br/>assessment included high-resolution ultrasonography to assess scar thickness and the <br/>patient and observer scar assessment scale (POSAS) for subjective scar characteristics <br/>assessment at the end of 3-month treatment, then after 6 months (as a follow-up). A <br/>mixed MANOVA was used to examine the therapy's impact on scar thickness and <br/>POSAS. Results: There was a significant decrease in scar thickness for the <br/>experimental group compared to the control group at post I and post II with effect sizes <br/>0.77 and 1.45 respectively. There was a significant decrease in OSAS (observer scar <br/>assessment scale) total score and general opinion for the experimental group compared <br/>to the control group at post I and post II with effect sizes 0.95, 2.02, and 0.53, 1.30 <br/>respectively. There was a significant decrease in PSAS (patient scar assessment scale) <br/>total score and general opinion for the experimental group compared to the control <br/>group at post I and post II with effect sizes of 0.89, 1.95, and 0.81, 1.39 respectively. <br/>Conclusion: BONT-A iontophoresis has a significant long-lasting effect on reducing <br/>HS thickness and improving its characteristics, with several advantages, as being non-<br/>invasive, painless, and externally controlled. |

| Summary, etc. | اشترك في هذه الدراسة 76 مريضا من الذين يعانون من الندبات التضخمية بعد الحروق. تراوح أعمارهم من ٢٠و ٤٠عاما. وتم تقسيمهم عشوائيا إلى مجموعتين. مجموعة (أ): تكونت من ٣٨ مريضا ممن تلقوا الإرحال الأيوني لسم البوتولينيوم النوع أ مرة شهريا لمدة تلاتة أشهر متتالية وتم اضافة برنامج تمرينات العلاج الطبيعي التقليدية مرتين أسبوعيا لمدة ثلاثة أشهر متتالية و المجموعة (ب): تكونت من ٣٨ مريض ممن تلقوا برنامج تمرينات العلاج الطبيعي التقليدية فقط مرتين أسبوعيا لمدة ثلاثة أشهر متتالية . تلقى المرضى من كلتا المجموعتين العلاج في الفترة من الشهر الثالث إلى الشهر السادس بعد التئام الحرق بعد الجراحة . خضعت كلتا المجموعتين لفترة متابعة ثلاثة أشهر من اخر جلسة. تلقت المجموعتين الرعاية الطبية اللازمة حتى التئام الحرق. استخدمت الموجات الصوتية عالية التردد لتقييم سمك الندبة قبل وبعد العلاج وبعد فترة المتابعة وأيضا استخدم مقياس تقييم الندب الملاحظ والمريض لتقييم خواص الندبات قبل وبعد العلاج وبعد فترة المتابعة. وتم استنتاج أن الإرحال الأيوني لسم البوتولينيوم النوع أ للندبات التضخمية بعد الحروق أكثرفاعلية من برنامج تمرينات العلاج الطبيعي التقليدية فقط.<br/> |
