Summary, etc. Background: Alzheimer's disease (AD) is a leading cause of dementia, characterized by progressive memory loss and cognitive decline, affecting millions globally. Early identification of cognitive markers in individuals at genetic risk is crucial for developing effective interventions.<br/>Objectives: This study aimed to investigate cognitive and executive functions in middle-aged offspring of patients with AD compared to healthy controls, examining the influence of the Apolipoprotein E (APOE) ε4 allele and APOE levels in plasma on these functions.<br/>Methods: This analytical observational, case-control study included 80 participants, divided into two groups: 40 offspring of AD patients (Group A) and 40 matched controls (Group B). Participants underwent comprehensive cognitive and executive function assessments, APOE genotyping, and plasma APOE level measurements.<br/>Results: The offspring group showed significantly lower performance on the Montreal Cognitive Assessment (MoCA) test, indicating early cognitive impairments despite being clinically asymptomatic. The presence of the APOE ε4 allele was higher in the offspring group, although no significant differences in plasma APOE levels were observed between the groups. A significant positive correlation was found between plasma APOE levels and cognitive performance measures.<br/>Conclusion: Middle-aged offspring of AD patients, even while cognitively normal, exhibit early cognitive vulnerabilities and a higher prevalence of the APOE4 genotype. These findings underscore the importance of early monitoring and potential interventions for individuals at genetic risk for AD.
