| Summary, etc. | Chronic subdural hematoma is one of the most common neurosurgical conditions. It is frequently encountered in elderly people, usually after minor head trauma. Recurrence of the hematoma is one of the most common complications which have been studied by many neurosurgeons. The recurrence and pathogenesis of CSDH are still a topic for debate and some uncertainty continues, the different techniques of drainage plays an important role in the recurrence of the hematoma, in this study, we compared 2 techniques of drainage of the hematoma between 2 groups: Two burr holes with subgaleal drain and embolization of the middle meningeal artery and their relation with the recurrence rates and improvement of the patients clinically and radiologically |

| Summary, etc. | يعد النزيف المزمن تحت الأم الجافية للمخ واحد من أكثر الأمراض الشائعة في جراحة المخ والاعصاب وكثيرا ما يصادف في كبار السن وعادة ما يحدث بعد إصابة بسيطة بالرأس.تعد الأشعة المقطعية فحصا أساسيا لمعرفة حجم ونوع ومكان النزيف وريحان الخط الناصف الناتج عنه مع استبعاد اي اصابات اخرى.ويعتبر الرنين المغناطيسى علي المخ اكثر دقة في تحديد النزيف متعدد الأقسام وتحديد وجود حواجز او أغشية لفيفة داخل النزيف.<br/>حديثا يعد حقن الشريان السحاقى الاوسط من الطرق الحديثة فى معاجلة النزيف المزمن تحت الأم الجافية وتميزت هذه الطريقة فى عدم وجود جروح فى الرأس أ والبقاء فترة أقل فى المستشفى وعدم وجود حالات إلتهابات أو حالات نزيف مرتجع. <br/>النزيف المرتجع يعد من المضاعفات الوارد بعد تفريغ النزيف المزمن تحت الأم الجافيه.<br/>إشتملت هذه الدراسة علي 20 مريضا يعانون من نزيف مزمن تحت الام الجافية حيث تم تقسيم المرضى إلى مجموعتين، مجموعة 1 تم تفريغ النزيف عن طريق ثقبين بالحفار اليدوي ووضع درنقه تحت الجلد ومجموعة 2 تم تفريغ النزيف عن طريق حقن شريان السحاق الاوسط وتمت ملاحظة المرضي فى المجموعتين لمتابعه التحسن الاكلينيكى و الاشعاعى و حدوث نزيف مرتجع تحت الأم الجافية.<br/>تم إجراء فحص عصبي كامل لجميع المرضي للكشف عن حدوث تحسن او تدهور في الحالة العصبية. <br/>تم عمل أشعه مقطعية على المخ بعد التدخل مباشرة وفى الشهر الثالث والسادس.<br/>أظهرت النتائج ان الفرق بين المجموعتين غير مؤثر، هذه النتائج رجحت بشدة ان حقن الشريان السحاقى الأوسط يعتبر أقل فى حدوث المضاعفات ألا وهى التهابات الجروح أو حدوث نزيف مرتجع واقل مدة بقاء فى المستشفى، بينما التدخل الجراحى يظل له اليد العليا فى معالجة الحالات الطارئة فى النزيف المزمن تحت الام الجافية. |
