| Summary, etc. | Background: Minimally invasive supracondylar extension osteotomy and above knee cast for management of fixed knee flexion deformity in CP diplegic patients. <br/>Aim and objectives: To assess the outcome, effectiveness and safety of a novel surgical technique in minimally invasive supracondylar extension osteotomy as a part of fixed knee flexion deformity in CP diplegic patients.<br/> Patients and methods: This was Randomized control trial study and was conducted at Abu- Elreesh Hospital, Cairo University, Cairo, Egypt on Patients of age 5 years till skeletal maturity from 2023 to 2024. A total of 20 patients were taken. The inclusion criteria were ambulatory children (GMFCS II and III). We identified patient age, sex, functional status, associated surgical procedures. Assessment was done clinically by using extensor lag test preoperative and postoperative. Radiological assessement done. Follow up period was (shortest 6 months and longest 12 months). <br/>Result:All patients included in this study showed improvement in extensor lag mechanism Mean from 15.79±5.68 SD to 4.21±1.3 SD after. Mean improved from 1.47±0.17 SD to 1.17 ±0.11 SD and remained at 6, 12 , 24 weeks and after . <br/>Conclusion: This technique in CP patients showed good results with minor complications and was not high technically demanding procedure. The study suggests that the procedure is safe and effective. We suggest that SCFEO is a better surgical procedure for spastic diplegic patients having fixed knee flexion deformity. |

| Summary, etc. | يعد تشوه ثني الركبة الثابت مشكلة شائعة تظهر لدى الأطفال والمراهقين والشباب المصابين بالشلل الدماغي التشنجي، والذي يتميز عادةً بثني الركبة المفرط. درجة انثناء الركبة والأعراض المرتبطة بها قد تتطور بسرعة بسبب الضغوط العالية على الركبة وفشل آلية بسط الركبة <br/>كان عدد الحالات 40 ركبة (20 مريضًا) مصابين بالشلل الدماغي والشلل المزدوج التشنجي. وكانت معايير الاشتمال الأطفال المتنقلة (GMFCS الثاني والثالث). حددنا عمر المريض، والجنس، والحالة الوظيفية، والإجراءات الجراحية المرتبطة بها. تم التقييم سريريًا باستخدام اختبار تأخر الباسطة قبل الجراحة وبعدها<br/>فترة تقييم المتابعة كانت (الحد الأدنى 6 أشهر والحد الأقصى 12 شهراً). أظهر جميع المرضى المشمولين في هذه الدراسة تحسنًا سريريًا في آلية تأخر الباسطة. بعد سنة واحدة من المتابعة بعد العملية الجراحية، كانت هناك تحسينات كبيرة في جميع المعلمات السريرية والقياسات الإشعاعية ودرجة GMFM. وتعكس هذه التدابير تحسين القدرة على الحركة الوظيفية، ولها تأثير إيجابي على نوعية الحياة.<br/>أثبتت هذه الدراسة أن قطع العظم التمدديفوق اللقميةلعظم الفخذ لحالات تشوه عظم الركبة الثابت في مرضى الشلل التشنجي المزدوج التشنجي هو وسيلة فعالة لتصحيح الركبة الثابتة، على الرغم من أن هناك حاجة إلى متابعة أطول للكشف عن التأثير الفعلي على هؤلاء المرضى.<br/>وبناءً على نتائجنا، نوصي بإجراء مزيد من الدراسات على المرضى الأكبر حجمًا وفترة متابعة أطول للتأكيد على استنتاجنا |
