Summary, etc. Although many studies to date have documented the efficacy of SDR worldwide on the treatment of spasticity in children, little work has been done to analyze the efficacy and outcomes of those techniques in our country. The goal of this study is analysis of the clinical & functional outcome of selective dorsal rhizotomy as a surgical treatment for spasticity in children with regard the evaluation of advantages, disadvantages and perioperative complication.<br/>Methods: <br/>After the neurosurgery department ethical committee approval, we prospectively reviewed all the children patients with spasticity (n=12) between January 2023 and August 2024 at Cairo University hospitals. Patient’s demographics, type of cerebral palsy ,Clinical presentation, Complication rates, Clinical and functional outcome with GMFS, Ashworth scale & gait score was recorded (Pre-operative ,immediate post- operative ,3 months post-operative and 6 months post-operative).<br/>Results: <br/>The mean age of the cases is 6 +/-2 in SD, 83.3 % of cases in the study were males while 16.7% of cases were females. Regarding cerebral palsy 91.7% of the children were diplegic cerebral palsy while 8.3% were quadriplegic CP. Regarding the surgical procedure 66.7% of cases operated upon laminoplasty from L1-L5 while the rest (33.3 %) operated upon laminoplasty from D12- L1.<br/>Regarding GMFS for cerebral palsy was assessed pre-operatively, immediate post-operatively, 3 months and 6 months post operatively and was compared showing statistical significance in the mean GMFS in the immediate post-operative and the follow up after 3 months and 6 months. (P-value < 0.001)<br/>Regarding Ashworth scale for Spasticity was assessed pre-operatively, immediate post-operatively,3 months and 6 months post operatively and<br/>was compared showing statistical significance in the mean Ashworth in the follow up after 3 months and 6 months. (P-value < 0.001)<br/>Conclusion:<br/>We concluded that Selective Dorsal Rhizotomy is the procedure of choice for treatment of spasticity in spastic diplegic CP and in selected children with spastic quadriplegic CP. Optimal selection and outcomes achieved using a multidisciplinary approach. In our study, SDR showed better outcome clinically and functionally with GMFS, Ashworth scale and gait score with lower rates of complication provided that all cases underwent early post-operative physiotherapy treatment. Serial and Long-term clinical follow-up are recommended with will organized physiotherapeutic program with cooperative and well oriented parents aiming for better outcome and to avoid relapse of symptoms.

Summary, etc. على الرغم من أن العديد من الدراسات حتى الآن قد وثقت فعالية جراحة القطع الانتقائي لجزور الاعصاب الطرفية في جميع أنحاء العالم لعلاج حالات الشلل التيبسي لدى الأطفال، إلا أنه لم يتم القيام إلا بالقليل من الدراسات لتحليل فعالية ونتائج تلك التقنيات في بلدنا ..<br/> لذا فان الهدف من هذه الدراسة هو تحليل النتائج السريرية والوظيفية للخيارات الجراحية المتاحة لحالات الشلل التيبسي لدى الأطفال فيما يتعلق بتقييم المزايا والعيوب والمضاعفات المحيطة بالجراحة..<br/>.<br/>بعد موافقة اللجنة الأخلاقية لقسم جراحة المخ والأعصاب، قمنا بمراجعة مستقبلية لجميع الاطفال الذين يعانون من الشلل التيبسي بين يناير 2023 وأغسطس 2024 بقسم جراحات المخ والاعصاب للاطفال بمستشفيات جامعة القاهرة. <br/>حيث تم تسجيل التركيبة السكانية للمريض، وتحديد نوع الشلل الدماغي، والاعراض السريرية، ومعدلات التحسن بعد الجراحة مباشرة وبعد 3 اشهر و 6 اشهر مع متابعة دقيقة للمضاعفات المحتملة مع متابعة النتائج السريرية والوظيفية بمقياس أشوورث وتحديد درجة المشي خلال تلك الفترات..<br/>استنتجنا أن جراحة القطع الانتقائي لجزور الاعصاب كخيار جراحي لعلاج الشلل التيبسي لدى الأطفال أظهر نتائج أفضل سريريًا ووظيفيًا مع مقياس أشوورث ودرجة المشي مع انخفاض بنسبة المضاعفات شرط أن تخضع جميع الحالات للعلاج الطبيعي المبكر بعد العملية الجراحية..<br/>كما يوصى بالمتابعة السريرية التسلسلية وطويلة الأمد من خلال برنامج العلاج الطبيعي المنظم مع أولياء الأمور المتعاونين والموجهين جيدًا بهدف تحقيق نتائج أفضل وتجنب انتكاس الأعراض.
