Summary, etc. Fibromyalgia (FM) is a complex disabling condition characterized by widespread muscular pain and soft tissue tenderness. FM is associated with other somatic complaints, physical disability, depressive episodes, and non-refreshed sleep, anxiety, cognitive dysfunction. <br/> It has been hypothesized that in FM there is activation of glia cells, mast cells and neutrophils which may be involved in pain amplification. The concept of FM as a pain amplification syndrome relies on the presence of generalized, multimodal allodynia and hyperalgesia and a dysfunction of endogenous pain inhibitory mechanisms. <br/> Furthermore, elevated concentrations of substances involved in nociceptive transmission and central sensitization, such as substance P (SP), glutamate, have been reported in the cerebrospinal fluid (CSF) of FM patients. <br/> Following activation, these cells release pro-inflammatory cytokines/chemokines such as tumor necrosis factor alpha (TNF-alpha), interleukin-1beta (IL-1b) and interleukin(IL-6). <br/> Oxidant/antioxidant balances were changed in FM. Increased free radical levels may be responsible for the development of FM. <br/> Sitagliptin is dipeptidyl peptidase (DPP-4) inhibitor. It increases the active form of incretins such as GLP-1 (glucagon-like peptide-1) or GIP (gastric inhibitory polypeptide). It is successfully applied in the treatment of type 2 diabetes mellitus (DM). Recently it is suggested that sitagliptin may have a beneficial effect on neurodegenerative disorders. It has anti-inflammatory effect, antioxidative and antiapoptotic properties by modifying glutamate and glutathione levels within different regions in the nervous system. <br/> Exercise is recommended as a Polypill for the control of many pathologies such as immune diseases, mostly those associated with infections and chronic low-grade inflammation as FM. Thus, therapeutic interventions via exercise could lower polymedication and its associated side effects in those patients. <br/> Exercise stimulates brain neuroplasticity and mitochondrial activity and gene expression in addition to inducing anxiolytic- and antidepressant-like effects. Exercise induces molecular and cellular changes in dorsal root sensory neurons and aids to improve regeneration after nerve injury as BDNF, NGF stimulate axonal growth. <br/>The present study was conducted to study the possible neurotherapeutic effects of sitagliptin, DPP-4 inhibitor(sitagliptin), exercise, the possible underlying mechanisms, as well as the combined effect of both in FM management. <br/> <br/>Thirty female albino rats, about six weeks old with nearly similar body weights ranging from 150 to 200 grams were used. Rats will be divided into the following 5 groups (6 rats / group): control, Reserpine induced fibromyalgia (RIF), sitagliptin treated group, exercise group and combined treated group. <br/> <br/> The behavioral tests are done on the 4th day to confirm RIF and at the end of study for assessment of treatment. at the end of study (4 weeks), nerve conduction velocity of right sciatic nerve, Inflammatory markers (IL 6, TNFα, IL 10, TGFβ) were measured. Brain, dorsal horn and Paw skin gene expression of: Substance P (SP), NGF, BDNF,oxidative stress markers ( SOD,GSH) and finally (serotonin, noradrenaline and dopamine) . <br/> <br/>At the end of the experimental period, RIF rats were presented with severe thermal hyperalgesia, motor incoordination and increased number of slips,as compared to the normal control group. <br/> <br/> Concerning biochemical parameters: RIF rats presented with high pro inflammatory markers and low anti-inflammatory markers level and increased pain-transmitting as SP.RIF rats exhibited decreased pain modulating peptides such as BDNF and NGF and reduced anti oxidative markers (GSH and SOD). In addition to decreased monoamines levels in brain, DHCs and paw skin. <br/> <br/>Treatment of RIF rats with sitagliptin or swimming exercise or combined therapy caused a significant improvement of all assessed parameters. <br/> Thus, we conclude that sitagliptin and exercise improved FM at different aspects; as they had anti-inflammatory, and antioxidant effects and pain modulating peptide effects. But higher neurotherapeutic effect was obtained when they combined.

Summary, etc. متلازمه الألم الليفي العضلي هو مرض يسبب تآكل وتدمير للأعصاب و يؤثر سل با علي حياة المرض ى. إن كيفية حدوث المرض وكيفية معالجت ه ما زال قيد البحث العلمي منذ سنوات . <br/> ف ي الدراسة الحالي ة ، حاولنا توضيح دور إعطاءعقار مثبط لإنزيم ثنائي الببتيد الببتيداز الرابع ) عقار سيتاجليبتين( )كعلاج دوائي( ، وممارس ة الرياضة)كجزء م ن نمط الحياة( ، وإدماجهما في علاج متلازمه الألم الليفي العضل ي . <br/>تم استخدام ثلاثين أنثى جرذان ألبينو ، عمرها حوال ي ست ة أسابيع بأوزان جس م متشابهة تقريبا تتراوح م ن 150 إلى 200 جرام. تم تقسيم الفئران إلى المجموعات الخمس التالي ة )6 فئران / مجموع ة(. <br/> المجموع ة الأولى : ) مجموعة التحكم الطبيعيه( <br/>المجموعه الثانيه : تم حقنها بماده الريزيربين تحت الجلد الذي يسبب أعراضا مماثله لمتلازمه الألم الليفي العضلي اليوم لمد ة ثلاثة أيام متتالية بجرعة نهائية قدرها 1 مل جم / كج م من وزن الجس م<br/> المجموع ة 3: )مجموع ة معالج ة بسيتاجليبتين( العلاج ب 11.67 ملغ/كغ من سيتاجليبتين لمدة 4 أسابيع<br/>المجموع ة 4: )مجموع ة التمرين( قامت الفئران بممارسة الرياض ة لمدة 1 ساع ة ف ي اليوم ، خمس ة أيام ف ي الأسبوع لمد ة 4 أسابيع. <br/> .المجموع ة 5: )الجمع بين كلا من سيتاجليبتين والتمرين (<br/> تم دراسة تأثيرتناول سيتاجليبتين وممارس ة الرياض ة على كل من:اعراض الإك ت ئاب ، فر ط إ لحساس، والإتزان الحركي، وسرعة توصيل الألياف العصبي ه. <br/>. كعلامات للإلتهاب في الدم (IL 6, IL10, TGFβ, 110, TGFβ, TNFα)تم قيا س <br/> ) SP, BDNF, NGF ( كما تم قياس المستوى الجيني لهذه للببتيدات المعدله للآلم. <br/> SOD,GSH)كعلامات مضاده للأكسده( .<br/>)Dopamine, serotonin and nor adrenaline( <br/> كمواد أحاديه الأمين في الدماغ والحبل الشوكي و جلد المخلب الخلف ي في المجموعات المختلفه. <br/> بعد 3 أيام من حقن الريسربين الذي يسبب الأل م العضلي الليفي تسبب في احداث فر ط التألم الحرار ي الشديد ، وعدم التناسق الحركي وزيادة عدد النزلقا ت للجزران مقارنة بمجموعة التحك م العادية. وبعد ذلك عولجت الفئران إم ا بسيتاجليبتين أو بتمارين السباح ة أ و مع مزيجهم<br/>أثبتت هذه الدراسة أ ن السيتاجلبتن و التمارين الرياضية له ا تأثيرات مضاده للاك ت ئاب ومضادة للإلتهابا ت ومضاد ة للأكسد ة وأفضل النتائج كانت عند دمجهما معا و قد تكون علاج مناسب لمتلازمه ا للألم الليفي العضل ي المستخلص العرب ي.<br/>يمكن الستنتاج من هذه الدراسة أ ن السيتاجليبتين والرياضة لهما أثار إيجابية ومضادة لإللتهاب ومضادة للأكسدة؛ وكان ت افضل النتائج عند دمج الثنين معا وحسنت بشكل كب ير متلازمه الالم الليفي العضلي.
