| Summary, etc. | Caesarean section (CS) is a procedure that involves the extraction of the baby from the mother’s abdomen for numerous fetal and maternal indications. The CS rate has increased dramatically in the last decade, reaching up to 32% in the USA (2014) and 38% (2011) at Assiut University in Egypt (Abdel-Aleem et al., 2013; Betran et al., 2016).<br/>Caesarean section is associated with many intra- and postoperative complications. Early complications include hemorrhage, bladder and bowel injuries, sepsis and postoperative ileus (Field and Haloob, 2016). <br/>Remote complications include the occurrence of intraperitoneal adhesions. The incidence of adhesions varies according to many factors, including the number of previous CSs, type of suture material, and homeostasis. Moreover, the incidence of adhesions after caesarean is between 12 and 75% (Berghella et al., 2013). <br/>The presence of adhesions in repeated CSs increases surgical difficulties and the incidence of bladder and bowel injuries (Van Der Krabben et al., 2002; Rahman et al., 2009).<br/>A multidisciplinary team should properly evaluate these patients before surgery. Few studies used ultrasound to evaluate the presence of adhesions before operation (Ayachi et al., 2018; Baron et al., 2018). <br/>However, they either used transvaginal ultrasound that may not be available or used a small sample size that does not accurately reflect the value of this tool. A novel technique was first described by Baron et al. (2018). <br/>Using the dynamic ultrasound sliding sign to predict presence of intraperitoneal adhesions in women with a repeated section. We classify the adhesions intra operative into mild, moderate, severe according to the operation time will be needed to deliver the baby and organ injury as bladder or bowel injury (Field and Haloob, 2016). |

| Summary, etc. | يعد تكوين الالتصاقات أمرا شائعا بعد الولادة القيصرية، ويزداد المدى والكثافة مع زيادة أعداد الولادات القيصرية المتكررة. تم اقتراح العديد من الاستراتيجيات للتنبؤ بوجود الالتصاقات، مثل؛ التاريخ الجراحي السابق، وشخصيات السطور الجاذبية، وتقييم ندبة الجلد. ومع ذلك، فإن هذه المحاولات تفتقر إلى التكاثر، وعادة ما يكون أخذ التاريخ غير الموثوق به هو الحال في العديد من المواقف<br/>تم استخدام اختبار العلامة المنزلقة الحوشية الموجه بالموجات فوق الصوتية لأول مرة من قبل الجراحين العامين في عام 1991 للكشف عن الالتصاقات داخل البطن. وقد أفيد بأن؛ يمكن ملاحظة الحركة المنزلقة للأحشاء تحت جدار البطن، الناتجة عن الاستنشاق العميق، بواسطة التصوير بالموجات الصوتية غياب هذه الحركة علامة انزلاق سلبية- يمكن استخدامها كعلامة مفيدة لوجود التصاقات داخل البطن.<br/>تهدف هذه الدراسة إلى تقييم فعالية علامة الانزلاق بالموجات فوق الصوتية في التنبؤ بوجود وشدة الالتصاقات داخل البطن لدى النساء اللائي يخضعن للعمليات القيصرية المتكررة.<br/>شملت هذه الدراسة المقطعية المستقبلية 90 امرأة حضرن المستشفيات في الثلث الثالث من الحمل المشار إليها لإجراء ولادة قيصرية متكررة اختياريا وتم فحصهن بحثا عن علامة منزلقة قبل الولادة القيصرية الاختيارية. تم تجنيد المشاركين في الدراسة (كلية طب قصر العيني، جامعة القاهرة).<br/>وكشفت النتائج الرئيسية للدراسة ما يلي:<br/> في دراستنا، عانى 3 (3.3٪) فقط من الأفراد من مضاعفات مع انخفاض مستوى الهيموجلوبين لديهم > 3 جم / ديسيلتر، بينما حدثت مضاعفات الضرر الحشوي في 4 (4.4٪) حالات.<br/> كشفت نتائجنا عن العديد من الارتباطات المهمة بين خصائص المريض ووجود التصاقات داخل البطن.<br/> في دراستنا، وجدنا أن النساء فوق سن 30 عاما لديهن نسبة أعلى بكثير من الالتصاقات مقارنة بالنساء الأصغر سنا <br/> في دراستنا، ارتبط عدد الولادات (>3) وزيادة عدد العمليات القيصرية السابقة بقوة بارتفاع معدل الالتصاقات (لكليهما.<br/> ومن المثير للاهتمام، وجدت دراستنا أن الولادات لأجل كانت مرتبطة بارتفاع معدل الالتصاقات مقارنة بالولادات المبكرة.<br/> في دراستنا، ارتبط وجود الالتصاقات بشكل كبير بانخفاض الهيموجلوبين >3 جم / ديسيلتر (p<0.021) والضرر الحشوي (p<0.007).<br/> في دراستنا، لوحظ انزلاق سلبي للرحم في 42 مريضا، في حين كان الانزلاق الإيجابي موجودا في 48 امرأة.<br/> في دراستنا، تم تأكيد الاشتباه في وجود التصاقات معتدلة أو شديدة داخل البطن في الجراحة في 33 (36.7٪)، وكان انتشار الالتصاقات المعتدلة أو الشديدة 36.7٪ (33/90)، بينما تم تأكيد انخفاض خطر الالتصاقات في 57 مريضا.<br/> النتيجة الأكثر لفتا للانتباه في دراستنا هي الدقة التشخيصية العالية للعلامة المنزلقة في التنبؤ بالالتصاقات داخل البطن مع حساسية 90.9٪، خصوصية 78.9٪، قيمة تنبؤية إيجابية 71.4٪، وقيمة تنبؤية سلبية 93.8٪، تشير نتائجنا إلى أن علامة الانزلاق هي طريقة موثوقة للتنبؤ بالالتصاقات.<br/> القيمة التنبؤية السلبية العالية (93.8٪) جديرة بالملاحظة بشكل خاص، لأنها تشير إلى أن علامة الانزلاق الإيجابية موثوقة للغاية في استبعاد الالتصاقات الكبيرة. <br/>أكدت هذه الدراسة أن اختبار العلامة المنزلقة هو وسيلة فعالة للكشف عن الالتصاقات داخل البطن لدى النساء اللواتي لديهن تاريخ من الولادة القيصرية.<br/>بناءً على نتائجنا، نوصي بإجراء مزيد من الدراسات على عينة من المرضى أكبر حجمًا وفترة المتابعة الأطول للتأكيد على استنتاجنا. |
