| Summary, etc. | Aim: This 2-arm parallel randomized clinical trial was to assess transverse dentoskeletal effects of Haas and Hyrax in patients with unilateral cleft lip and palate (UCLP) in mixed dentition. In addition, anteroposterior, vertical, dental and periodontal effects of the two appliances were assessed. <br/>Methodology: Twenty-four patients with posterior crossbite were randomly divided into; Haas group with average age of 9.5±1.24 years and Hyrax group with average age of 9.7±1.26 years. Pre-expansion CBCT was taken. Activation regimen was 2 turns twice per day. Both appliances were left as retention appliances for 6 months after expansion then post-retention CBCT was taken. Eligibility criteria included patients with UCLP, erupted upper first molars, periodontally sound teeth and no previous orthodontic treatment. Measurements of the CBCT were done on Anatomage version 6.3 software to evaluate the effects of both appliances.<br/>Results: Four patients were lost in the follow up. Both expanders produced significant alveolar crest expansion by (2.76 ± 1.51) mm and (1.92 ± 1.77) mm respectively. Hyrax produced significant maxillary skeletal expansion by (3.64 ± 3.21) mm. Haas caused significant posterior and anterior dental expansion by increasing the intermolar and the intercanine widths by (3.92 ± 1.94) mm and (3.5 ± 2.99) mm respectively while only posterior dental expansion was achieved by Hyrax by (3.62 ± 3.22) mm intermolar width increase. Both appliances increased the anterior facial height significantly by (2.41± 1.79) mm in the Haas group and (2.47± 2.53) mm in the Hyrax group. Hyrax caused decrease in mesio-buccal bone plate thickness of the upper first molar on the non-cleft side but this was not clinically relevant as the mean decrease in buccal bone plate thickness was about 0.5mm.<br/>Conclusion: Both appliances corrected the posterior crossbite by alveolar crest expansion and posterior dental expansion with increasing the anterior facial height and without any clinically significant periodontal side effects. Hyrax caused more maxillary skeletal expansion while Haas produced more anterior dental expansion. |

| Summary, etc. | كان هدف هذه الدراسة هو تقييم التاثيرات السنية – العظمية العرضية لاثنين من أشهر أجهزة التوسيع السريع للفك العلوي و هما ال " هاس" و ال " هايركس" في مرضي الشفة الأرنبية و شق سقف الحلق في ناحية واحدة في وقت مرحلة الأسنان المحتلطة. و تمت أيضا دراسة التأثيرات الأمامية-الخلفية, و النأثيرات الرأسية, و التأثير علي الأسنان و علي اللثة.<br/>تم ضم أربعة و عشرين حالة من حالات الشفة الأرنبية و شق سقف الحلق في ناحية واحدة الذين يعانون من عضة عكسية خلفية, و تم تقسيم الحالات عشوائيا لمجموعتين متساويتين : مجموعة الهاس و متوسط العمر بها (9.5±1.24) مم و مجموعة الهايركس و متوسط العمر بها (9.7±1.26). تم أخذ اشعة مقطعية ثلاثية الأبعاد للأسنان قبل تركيب الجهاز ثم بعد أن انتهي الجهاز من توسيع الفك تـم تركه لمدة ستة أشهر في فم المريض كجهاز تثبيت. بعد ستة أشهر, تم أخذ أشعة مقطعية ثانية.تم قياس الاشاعات علي يرنامج أناتوماج النسخة السادسة.<br/>وضح التحليل الاحصائي أن الجهازين نجحا في توسيع المنطقة الحاملة للأسنان في الفك العلوي بمقدار(2.76 ± 1.51) مم للهاس و (1.92 ± 1.77) مم للهايركس. نجح الهايركس فقط في توسيع عظم الفك العلوي بمقدار(3.64 ± 3.21) مم. ادي الجهازين الي زيادة الطول الأمامي للوجه بمقدار(2.41± 1.79) مم للهاس و(2.47± 2.53) مم للهايركس.<br/>نجح الهاس في التوسيع الخلفي للأسنان بزيادة المسافة بين الضروس الأولي الدائمة بمقدار(3.92 ± 1.94) مم و نجح أيضا في التوسيع الأمامي للأسنان بزيادة المسافة بين الأنياب اللبنية بمقدار(3.5 ± 2.99) مم بينما نجح الهايركس فقط في التوسيع الخلفي للأسنان بزيادة المسافة بين الضروس الأولي الدائمة بمقدار(3.62 ± 3.22) مم.أدي جهاز الهايركس الي تقليل سمك العظم عند الجزء الامامي الخارجي من الضرس الاول الدائم و لكن هذا الرقم لا يمثل مشكلة اكلينيكية لان القيمة 0.5 مم فقط.<br/>و لذلك نجح الجهازين في علاج العضة العكسية الخلفية عن طريق توسيع المنطقة الحاملة للأسنان و عن طريق التوسيع الخلفي للأسنان بزيادة المسافة بين الضروس الأولي الدائمة و في نفس الوقت تسببا في زيادة الطول الأمامي للوجه مع عدم وجود أي اثار جانبية لثوية ذات اهمية اكلينيكية لأي من الجهازين. نجح أيضا جهاز الهايركس في توسيع عظم الفك العلوي بينما نجح جهاز الهاس في التوسيع الأمامي للأسنان بزيادة المسافة بين الأنياب اللبنية. |
