| Summary, etc. | Background: Coronavirus disease (COVID-19) outbreak, caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), led to a series of acute atypical respiratory diseases. The host immune response to SARS-CoV-2 appears to play a critical role in disease pathogenesis and clinical manifestations through activation of uncontrolled inflammatory responses characterized by marked pro-inflammatory cytokine release, e.g. interleukin (IL)-6, tumor necrosis factor (TNF)-α, etc., leading to “cytokine storm” and hence, tissue damage. Cardiovascular injuries, including myocarditis, cardiac rhythm abnormalities, endothelial cell injury, thrombotic events and even acute heart failure, are observed in some patients with COVID-19.<br/>We aimed to study the occurrence of cardiac abnormalities that may happen in severe COVID-19 infections and their early detection by measuring blood levels of coenzyme Q10, N-terminal pro-brain natriuretic peptide (NT-ProBNP) and performing conventional transthoracic echocardiography.<br/>Methodology: Sixty COVID-19 patients (thirty severe cases requiring ICU admission and thirty mild home-isolated cases) were compared to thirty healthy age and sex matched controls. The whole study population was subjected to laboratory measurement of blood level of coenzyme Q10 and NT-ProBNP and performance of conventional transthoracic echocardiography. <br/>Results: There was statistical significance between the three study groups regarding ejection fraction (EF), tricuspid annular plane systolic excursion (TAPSE) and N-terminal pro-brain natriuretic peptide (NT-ProBNP) (p-value: <0.001, 0.001 and <0.001 respectively). Coenzyme Q10 level showed no statistical significance between the three study groups (p-value: 0.496). A cut-off of 960.9 pg/mL NT-ProBNP is desired to predict the likeliness of ICU admission in COVID-19 patients with sensitivity: 73% and specificity: 90%. The presence of statistically significant negative correlation between NT-ProBNP and EF in whole study population, COVID-19 patients, ICU-admitted COVID-19 patients and controls (r-value: -0.658, -0.674, -0.709 and -0.493 respectively and p-value: <0.001, <0.001, <0.001 and 0.006 respectively). The presence of statistically significant negative correlation between NT-ProBNP and TAPSE in whole study population, COVID-19 patients, ICU-admitted COVID-19 patients and controls (r-value: -0.539, -0.460, -0.514 and -0.617 respectively and p-value: <0.001, <0.001, 0.004 and <0.001 respectively). There was a statistically significant negative correlation between coenzyme Q10 and interleukin-6 (r-value: -0.391 and p-value: 0.032).<br/>Conclusion: Subtle cardiac abnormalities that may happen as a result of COVID-19 infection can be detected by measuring blood levels of NT-ProBNP and performing an echocardiography as a screening tool to all COVID-19 positive patients. However, it is not recommended to measure plasma levels of coenzyme Q10 in COVID-19 patients. |

| Summary, etc. | اجريت دراسة تحليلية ملاحظة على مجموعة الحالات و المجموعة الضابطة على 90 شخص بالغ مقسمين على ثلاث مجموعات:<br/>المجموعة الاولى: مرضى كوفيد 19 الذين احتاجوا الى دخول وحدة العناية المركزة وعددهم 30.<br/>المجموعة الثانية: مرضى كوفيد 19 الذين تم عزلهم بالمنزل و عددهم 30.<br/>المجموعة الثالثة: اشخاص اصحاء مماثلين فى العمر و الجنس و عددهم 30.<br/>تم اختيار حالات المجموعة الاولى من المرضى المحجوزين بوحدة العناية المركزة بقسم 6 – مستشفى القصر العينى وحالات المجموعة الثانية من المرضى المترددين على عيادة كوفيد 19 بالعيادات الخارجية بمستشفى القصر العينى. مجموعة الثالثة الضابطة تم اختيار الاشخاص من المترددين على عيادة الجراحة بالعيادات الخارجية بمستشفى القصر العينى.<br/>خضع جميع الافراد لأخذ التاريخ المرضى الكامل، وتم عمل فحص موجات فوق صوتية على القلب واختبار مستوى مساعد الانزيم كيو10 والبيبتيد الدماغى المدر للصوديوم فى الدم لكل الافراد فى مجموعات الدراسة لتحديد مدى كفاءة وظائف عضلة القلب الانقباضية.<br/>كان هناك فروق ذات دلالة إحصائية بين مجموعات الدراسة من حيث مستوى البيبتيد الدماغى المدر للصوديوم فى الدم (اعلى فى مجموعة الحالات من المجموعة الضابطة)<br/>اظهرت الدراسة فروق ذات دلالة إحصائية بين المجموعات من حيث مقاييس فحص الموجات فوق الصوتية على القلب حيث اظهرت مجموعة الحالات وجود انخفاض فى اداء البطينين (الايسر والايمن) بوظائف الانقباض مقارنة بالمجموعة الضابطة.<br/>لم نجد أي فروق ذات دلالة إحصائية بين مجموعات الدراسة فيما يتعلق بمستوى مساعد الانزيم كيو10 فى الدم.<br/>أظهرت الدراسة وجود علاقة واضحة بين شدة الاصابة بكوفيد 19 وحدوث قصور فى وظائف عضلة القلب عن طريق فحص الموجات فوق الصوتية على القلب من ناحية و ارتفاع مستوى البيبتيد الدماغى المدر للصوديوم فى الدم من ناحية اخرى.<br/>هذا يستوجب ضرورة عمل فحص موجات فوق صوتية على القلب وقياس مستوى البيبتيد الدماغى المدر للصوديوم فى الدم لكل مرضى كوفيد 19 حتى نتمكن من التشخيص المبكر لاعتلال عضلة القلب قبل ظهور الاعراض المرضية مما يتيح سرعة علاج هؤلاء المرضى. |
