| Summary, etc. | Introduction: In cases of long-term teeth loss and significant sinus pneumatization, minimizing the remaining height of the posterior maxillary ridges makes replacing lost teeth with a permanent prosthesis more difficult. Several approaches have been published to deal with the decreased vertical bone height in delayed or simultaneous implant placement. Sinus floor elevation using a lateral technique in conjunction with bone grafting has been shown to have high long-term results.<br/> Aim: This study aims to evaluate the bone gain after sinus floor elevation simultaneously with implant placement using autogenous chin bone ring versus autogenous sticky bone grafting techniques. <br/> Methodology: Twenty patients were selected from the oral and maxillofacial surgery outpatient clinic of the academic hospital of the Faculty of Dentistry, Cairo University-Egypt. A total of thirty implants were placed with sinus augmentation. Seventeen implants were placed simultaneously with the chin bone ring (study group) and fourteen implants were placed simultaneously with sticky bone (control group) with a lateral sinus lift approach. Sinus lift procedures for both groups were done under general anesthesia. The bone gain in the study group and the control group were evaluated immediately and 6 months postoperatively by CT and Using 3D Slicer (www. slicer. org), volume measurements were carried out using manual segmentation of the sinus area and/or graft by painting slice by slice on each axial image finally with the help of the (segment statistics module) of the software, the volume of the segmented area was automatically measured in cm3.<br/> Results: The study was conducted on 20 cases randomly and equally allocated to each studied group (i.e., 10 cases each). In the control group, there was a significant increase in measured sinus volume after 6 months (p<0.001). In contrast, the study group's measured sinus volume decreased significantly after 6 months (p=0.029). There was a significant difference in the change of sinus volume between both groups, with the volume in the control group increasing and, in the study, group decreasing after 6 months (p<0.001).<br/> Conclusions: Overall, we can conclude that the autogenous chin bone ring was a reliable technique, although it was technique-sensitive with a high rate of success for augmenting the atrophic posterior maxillary ridge three-dimensionally with simultaneous implant placement. It improves the osseointegration of both bone ring and implant together, gives us a bone gain, maintains the bone volume, and resists air creep of sinus, thus reducing the period of treatment, if compared with any other technique using autogenous bone graft, from the time the patient decides to restore a tooth till the implant installation in case of large sinus pneumatization and remaining residual ridge less than 4mm.<br/> |
