Summary, etc. Background: Diabetes mellitus is a serious metabolic disorder in which microvascular complications, <br/>such as peripheral neuropathy, retinopathy, and nephropathy, regularly occur but may remain <br/>undetected in daily practice. S100A4 has recently been reported as a novel biomarker and an important <br/>regulator of glioma stem cells, with its increased expression contributing to the appearance of a <br/>metastatic phenotype, as well as having been described as a marker for lupus nephritis activity, a <br/>determinant factor for the onset of the complex inflammatory autoimmune disease lupus <br/>erythematosus; Aim of work: Our study aimed to assess the potential value of S100A4 gene <br/>polymorphism as a diagnostic biomarker of nephropathy in type 2 diabetic patients and to assess <br/>the association between S100A4 gene polymorphism and carotid intima media thickness; Patients <br/>and Methods: This is observational case-control study included 150 patients with type 2 diabetes <br/>and 50 age- and sex-matched healthy controls, aged 40 to 70 classified into four groups: Group I: <br/>50 diabetic patients no albuminuria. Group II: 50 diabetic patients with microalbuminuria. Group <br/>III: 50 diabetic patients with macroalbuminuria. Group IV: 50 healthy controls; Results: No <br/>significant differences in gender and age among the groups. However, BMI, DM duration, and <br/>blood pressure were significantly higher in diabetic groups, especially in patients with <br/>macroalbuminuria. Fasting blood sugar, 2-hour postprandial sugar, and HbA1c levels were <br/>significantly elevated in diabetic groups. Serum creatinine and urea levels also showed significant <br/>differences, indicating renal involvement in diabetes progression. The study found a significant <br/>correlation between S100A4 levels and the severity of albuminuria in diabetic patients, <br/>highlighting its potential as a biomarker; Conclusion: The study examined 150 type 2 diabetes <br/>mellitus (T2DM) patients and 50 controls, revealing significant differences in clinical, laboratory, <br/>and genotypic parameters.

Summary, etc. مرض السكري هو اضطراب ايضى خطير تحدث فيه مضاعفات الأوعية الدموية الدقيقة، مثل الاعتلال العصبي المحيطي واعتلال الشبكية واعتلال الكلي، ولكنها قد تظل غير مكتشفة في الممارسة اليومية.<br/>اعتلال الكلي السكري هو أكثر أمراض الكلى المزمنة انتشارًا وهو السبب الرئيسي لأمراض الكلى في المرحلة النهائية في جميع أنحاء العالم. ويرجع ذلك في الغالب إلى تزايد الإصابة بمرض السكري من النوع الثانى المرتبط بالسمنة.<br/> مثل أمراض الكلى المزمنة الأخرى، يتم تصنيف التطور لاعتلال الكلي السكري وفقًا لمستوى إفراز ألبومين البول وانخفاض معدل الترشيح الكبيبي.<br/>في مرضى السكري، يوصف تطور الزلال الدقيق فى البول بأنه اعتلال الكلي الأولي، في حين أن وجود الزلال الكبير في البول يوصف بأنه اعتلال الكلي العلني، ويعتبر معدل الترشيح الكبيبي أقل من 15 مل/دقيقة/1.73 م2 فشل كلوى والجدير بالذكر أن الإصابة الكلوية تتقدم في حوالي ثلث المرضى الذين يعانون من اعتلال الكلي السكري ويمكن أن يعاني بعض المرضى من زلال البول الدقيق أو اعتلال الكلي العلني أو كلاهما معا الذي يرتبط عادة بتحسين السيطرة على مرض السكري وارتفاع ضغط الدم مع الأدوية.<br/>بالنسبة لدور الالتهاب في اعتلال الكلي السكري سابقًا في سياق النظريات الحالية التي تركز على الالتهاب وتنشيط الأرومة الليفية مثل الانتقال الظهاري إلى الوسيط والانتقال البطاني إلى الوسيط .<br/>إن سمك الوسائط الداخلية للشريان السباتي المشترك هو مؤشر قابل للقياس لوجود بعض التشوهات.
