Summary, etc.	Background: Pulmonary embolism (PE) is a common and potentially fatal cardiovascular disorder that must be promptly diagnosed and treated. The incidence of PE has apparently increased in the last decades, due to improvements in diagnostic pathways.<br/>Objective: To assess the degree of implementation of guidelines in the diagnosis and management of PE. The prevalence of PE among admitted patients, their demographic data, modes of presentation, management strategies and in-hospital outcomes were also assessed. <br/>Methods: Patients ≥ 18 years admitted to hospital with PE were evaluated regarding their demographic data, comorbidities, risk factors, mode of presentation, investigation findings, management strategies and outcomes during their hospital stay.<br/><br/>Results: 200 patients were enrolled in the duration from March 2023 to April 2024. The incidence of PE was 200/8648 patient which represented 2.3% of ED visits during this period. The mean age in our study was 56.7 ± 9.8 years, females comprised 58% of patients. The most common mode of presentation at the time of diagnosis was dyspnea in 64.5% of patients. Regarding risk factors, bed rest for >3 days was the most common risk factor in 32% of patients followed by previous VTE in 24% of patients. 95.5% of the patients underwent CT angiography and it was positive in all studies. The initial treatment of patients with VTE in the ED basically involved monotherapy with UFH in 63% while only 8.5% of patients received direct oral anticoagulants. Mortality occurred in 3% of patients with 1% complicated by life-threatening bleeding.<br/>Conclusion: Pulmonary embolism (PE), despite being common, often remains elusive as a diagnosis, and clinical suspicion needs to remain high while seeing a patient with cardiopulmonary symptoms. Risk stratification with clinical scores, biomarkers and imaging help to refine the best treatment strategy.

Summary, etc.	الانسداد الرئوي الحاد هو مرض شائع وقد يكون قاتلاً خاصة في الشهر الأول ويمكن أن يتنوع الانسداد الرئوي في شدته من عدم وجود أعراض إلى الوفاة المفاجئة، ويبدو أن معدل حدوث الانسداد الرئوي قد زاد في العقود الأخيرة، بسبب التطور في طرق التشخيص والتوافر الواسع للآشعة المقطعية، بالإضافة إلى زيادة الوعي بالمرض.<br/>وقد قدمت الجمعية الأوروبية لأمراض القلب إرشادات العلاج بناء على خطورة حالة المريض كما تغيرت أساليب علاج الانسداد الرئوي بشكل كبير على مدى العقد الماضي، وقد تأثرت بشكل ملحوظ بإدخال العلاجات المباشرة المضادة للتخثر وقد سمحت سهولة استخدامها، وعدم الحاجة إلى مراقبة مخبرية روتينية، وانخفاض معدلات النزيف بقبول أكبر من المرضى مقارنةً بأدوية مضادات التجلط التقليدية.<br/>وقد كان لتمديد مدة العلاج في بعض المرضى الذين يعانون من الانسداد الرئوي تأثير كبير في تقليل خطر التجلط الوريدي المتكرر، كما كان لحساب درجات الاحتمالية وتحليل (دي دايمر) D-dimer دور في التقليل من الافراط في الفحوصات أو العلاج غير اللازم لبعض المرضى.<br/>وقد هدفت هذه الدراسة تقييم الالتزام بالإرشادات العيارية لعلاج السدة الرئوية الحادة من خلال خبرة مركز مرجعي منفرد ولقد وجدنا أن متوسط عمر المرضى كان 56.7±9.8 منهم 58% من الإناث والشكوى الأساسية كانت ضيق بالتنفس، كما كانت ملازمة الفراش لمدة أطول من ثلاثة أيام أعلى عامل خطورة للإصابة بالسدة الرئوية الحادة وقد تم عمل أشعة مقطعية بالصبغة على الشرايين الرئوية في 95.5% من المرضي، و تلقى 63% من المرضى علاج مبدأي بعقار الهيبارين ، وتلقى 8.5% فقط من المرضى العلاجات المباشرة المضادة للتخثر وقد حدثت الوفاة في 3% من المرضي كما عاني 1% من المرضي من نزيف شديد.<br/>وعلى الرغم من التقدم الذي تحقق على مدى العقد الماضي، إلا أن الانسداد الرئوي لا يزال له عواقب طويلة الأمد وهامة على المرضى، بما في ذلك ضيق التنفس المزمن، وتأثير ذلك على جودة الحياة وهناك حاجة إلى المزيد من الأبحاث المستقبلية لتحديد التدخلات والدعم المناسبين.
