Objectives: To investigate the effect of thoracic manipulation (TM) versus myofascial release (MR) by using instrument assisted soft tissue mobilization (IASTM) on pain intensity, pain pressure threshold, range of motion (ROM), proprioception, and disability in patients with shoulder impingement syndrome (SIS). Design: A single blinded, parallel groups randomized controlled trial.<br/>Setting: Outpatient Physical Therapy clinics, elshiekh zayed specialized hospital and misr Petroliom Company. Participants: Sixty patients, 25-40 years old with shoulder impingement syndrome were randomly assigned into three equal groups. Intervention: Group (A) received scapular stabilization exercises only, group (B) received scapular stabilization exercises with thoracic manipulation, and group (C) received scapular stabilization exercises with MR by IASTM techniques. The intervention was received as 3 sessions a week for 4 weeks. Main Outcome Measures: Visual analogue scale (VAS), pain pressure threshold by algometer, ROM by kineovea sofware, proprioception by isokentic device, and Disability of arm, shoulder and hand questionnaire (DASH) were evaluated before treatment, after 4 weeks of treatment, then after one month without intervention (follow up). Results: The results showed statistically significant differences among groups regarding pain intensity, pain pressure threshold, shoulder flexion, abduction, external rotation ROM, joint position error, and DASH outcome measures (P>0.05).The mean and standard deviation (M±SD) of VAS were 62.5±8.51, 55.0±6.88, and 45.0±10.0mm for Group (A), (B) and (C), respectively with F= 21.05and P<0.001. The (M±SD) of Pain pressure threshold were 3.1±0.3, 3.28±0.15, and 3.5±0.2kg/cm2 for Group (A), (B) and (C), respectively with F= 16.16and P<0.001. The (M±SD) of DASH were 71.15±5.74, 66.45±5.21, and 61.7±6.21for Group (A), (B) and (C), respectively with F= 13.58and P<0.001. Conclusion: Adding MR by IASTM versus TM to scapular stabilization exercises had better improvement in favor of MR, on pain, range of motion, proprioception, and function of shoulder joint than scapular stabilization exercises alone in patients with SIS. It is recommended to use IASTM as adjunctive therapy in treating subjects with shoulder impingement syndrome. |

الهدف: للتحقق من تأثير تحريك فقرات الظهر مقابل الانفراج الليفي العضلي بواسطة أداة تساعد في تعبئة الأنسجة الرخوة على شدة الألم، الحد الاقصى لضغط الألم، المدىالحركى، و الحس العميق والإعاقة في مرضى متلازمة انحشار الكتف. <br/>التصميم: مجموعة غير مرئية، متوازية جروبات ذات دراسة عشوائية محكمة.<br/>الإطار: قسم العيادات الخارجية للعلاج الطبيعى.<br/>المشاركون: ستون مريضا ،تتراوح أعمارهم بين 25-40 عاما ممن يعانون من متلازمة انحشار الكتف تم تقسيمهم بشكل عشوائي إلى ثلاث مجموعات متساوية.<br/>التدخل:المجموعة(أ) تلقت تمارين تثبيت كتفي فقط، المجموعة (ب) تلقت تمارين تثبيت كتفي معتمارين تحريك فقرات الظهر و المجموعة(ج) تلقت تمارين تثبيت كتفي مع تمارين الانفراج الليفي العضلي بواسطة أداة تساعد في تعبئة الأنسجة الرخوة. تم العلاج ثلاث جلسات اسبوعيا لمدة 4 أسابيع.<br/>مقاييس النتائج الرئيسية: تم تقييم المقياس التمثيلى البصرى، الحد الاقصى لضغط الألم، المدى الحركى، الحس العميق، واستبيان إعاقة الذراع والكتف واليد قبل العلاج وبعد أربعة أسابيع من العلاج وبعد شهر من عدم العلاج .<br/>النتائج:أظهرت النتائج فروق ذات دلالة إحصائية بين المجموعات فيما يتعلق بشده الألم، الحد الاقصى لضغط الألم، ارتفاع الكتف، ، والدوران الخارجي، وخطأ موضع المفصل، ومقاييس نتائج استبيان اعاقات الذراع والكتف واليد (P > 0.05). كان المتوسط والانحراف المعياري (M ± SD) لالم ـ هو 62.5 ± 8.51 و55.0 ± 6.88 و45.0 ± 10.0 مم للمجموعة (أ) و(ب) و(ج) ، على التوالي مع F = 21.05 و P <0.001. كانت (M ± SD) الحد الاقصى لضغط الألم 3.1 ± 0.3 و 3.28 ± 0.15 و 3.5 ± 0.2 كجم / سم 2 للمجموعة الأولى والثانية والثالثة على التوالي مع F = 16.16 و P <0.001. كانت (M ± SD) لـ استبيان اعاقات الذراع والكتف واليد71.15 ± 5.74 و 66.45 ± 5.21 و 61.7 ± 6.21 للمجموعة الأولى والثانية والثالثة على التوالي مع F = 13.58 و P <0.001.<br/>الاستنتاج: إن إضافة برامج الانفراج الليفي العضلي بواسطة اداه تساعد في انفراج الانسجه الرخوه مقابل تمارين تحريك الفقرات الصدريه إلى تمارين التثبيت الكتفي كان له تحسن أفضل لصالح الانفراج الليفي العضلي، فيما يتعلق بشدة الألم والمدي الحركي واستقبال الحس العميق ووظيفة مفصل الكتف مقارنة بتمارين التثبيت الكتفي وحدها في مرضي متلازمه انحشار الكتف. يوصى باستخدام اداه الانفراج الليفي العضلي كعلاج مساعد في علاج الأشخاص الذين يعانون من متلازمة انحشار الكتف. |
