| Summary, etc. | Aim: The aim of this study was to compare the effect of three different oral hygiene protocols on the amount of S. mutans colonization in adult patients with fixed orthodontic appliance.<br/>Materials and methods: A total of 27 subjects who fulfilled the eligibility criteria were randomly allocated in the study. The subjects were randomly allocated into 3 groups, i.e., Group I (conventional manual toothbrushing with fluoridated toothpaste), Group II (Conventional manual toothbrushing combined with interdental brushes) and Group III (Conventional manual toothbrushing combined with interdental brushes and chlorhexidine mouthwash). Saliva samples were collected from each subject on the day of bonding (T0) and 1 month after bonding (T1), then sent for microbial analysis to calculate S. mutans viable count (CFU/ml). The baseline data including plaque index (PI) and gingival index (GI) were recorded on the day of bonding (T0) and one month later (T1).<br/>Results: T1 vs. T0: Group I showed significant increase in S. mutans viable counts (P = 0.0117) and no significant changes in PI scores (P-value = 0.192), Group II showed no significant changes in S. mutans levels and plaque index (P = 0.425) (P-value = 0.110) respectively, while Group III showed significant decrease in S. mutans viable counts and plaque index (P = 0.0078) (P-value = 0.050) respectively. Upon comparing the three oral hygiene protocols, both group II and group III exhibited prominent impact on oral hygiene whether by maintaining or reducing S. mutans levels respectively. The inter-group PI results showed significant difference (P-value = 0.018), where Group I showed the highest percentage increase in PI and Group II showed lower percentage increase, however difference in percentage change between them was statistically insignificant (P-value= > 0.999). Group III showed a noticeable decrease in PI scores by ~ 37% that was statistically significant compared to PI percentage of group I and II with P-value= 0.0448 and 0.0412, respectively. Regarding the GI, there was no significant changes between the three different oral hygiene protocols both intra and inter-groups.<br/>Conclusion: Conventional manual toothbrush combined with interdental brush protocol with or without CHX were good protocols for improving oral hygiene in orthodontic patients, while the protocol of using conventional manual toothbrushes alone was the least effective oral hygiene protocol. Chlorhexidine mouthwash can also be used in some orthodontic patients with periodontal problems, as it showed significant plaque index reduction by reducing the amounts of plaque deposition on teeth. |

| Summary, etc. | يعتبر لعلاج تقويم الأسنان الثابت مزايا وعيوب مثل العديد من العلاجات الأخرى. من بين السلبيات زيادة رواسب الأغشية الحيوية البكتيرية ، وتغيير الجراثيم الفموية وزيادة خطر الإصابة بـآفات البقع البيضاء النخرية. إن تكوين الحاصرات التقويمية ، و سلك الربط ، والسلاسل المرنة والأربطة المستخدمة في علاج تقويم الأسنان ، تحد من قدرة اللعاب على التنظيف الذاتي وتوفر ملاجئ للبكتيريا المختلفة التي تزيد بالتالي من كمية تراكم الجير وتقلل بسرعة من درجة حموضة الفم مما يؤدي إلى إزالة المعادن من المينا وتطور آفات البقع البيضاء النخرية. تعتبر بكتيريا العقدية الطافرة المستعمر الرئيسي في انواع البكتيريا المختلفة الموجودة بالترسبات الجيرية.<br/>وبالتالي، كان الهدف من دراستنا هو إيجاد بروتوكول فعال لنظافة الفم للحفاظ على صحة الفم جيدة أثناء العلاج التقويمي الثابت من خلال تقييم فعالية الاستخدام الإضافي للفرشاة المخصصة للاستخدام بين الأسنان وغسول الفم المحتوي على مادة كلورهيكسيدين لتقليل كمية تكوين الأغشية الحيوية البكتيرية وعدد طافرات بكتيريا العقدية الطافرة في مرضى التقويم الثابت.<br/>تم إجراء تجربة سريرية عشوائية على ٢٧ مريضًا تم ضمهم في هذه الدراسة ؛ تتراوح أعمارهم بين ١٨ إلى ٣٠ عامًا ولديهم أسنان دائمة كاملة باستثناء ضروس العقل ، ولم يستخدمو أي نوع من أنواع غسول الفم خلال الشهر الماضي ، ومع نظافة الفم الجيدة إلى المعتدلة مستدلا باللثة السليمة غير الملتهبة وعدم وجود أي آفات نخرية. تم تعيين ٩ مرضى للمجموعة الأولى التي استخدمت فرش الأسنان اليدوية التقليدية اللينة مع معجون أسنان يحتوي على فلوريد. تم تعيين بقية المرضى لمجموعتين ، ٩ لكل مجموعة. في المجموعة الثانية ، تمت إضافة فرش ما بين الأسنان إلى البروتوكول المستخدم في المجموعة الأولى وفي المجموعة الثالثة تمت إضافة غسول الفم الكلورهيكسيدين بنسبة ٠.١٢٥٪ إلى البروتوكول المستخدم في المجموعة الثانية.<br/>تم جمع عينات اللعاب مرتين في مواعيد محددة. مباشرة قبل الصاق الحاصرات التقويمية (T0) وبعد أربعة أسابيع من التركيب (T1) ، وارسالها للتحليل الميكروبي وقياس عدد طافرات بكتيريا العقدية الطافرة في شكل وحدات تشكيل مستعمرة (CFU).<br/>تم قياس مؤشرات الجير واللثة باستخدام طريقة قياس Silness and Löe. تم تحديد مؤشر الجير لكل مريض من خلال الأسنان المرجعية فقط (أرقام الأسنان: 16 ، 12 ، 24 ، 36 ، 32 ، 44) عن طريق تمرير مسبار على طول أسطح الأسنان (الدهليزى واللساني والإنسي والوَحشيّ) على حد سواء فوق وتحت مستوى اللثة و تسجيل درجة لكل سطح من ٠-٣. أعطيت مؤشرات اللثة أيضًا درجة ٠-٣ وفقًا لنظام الدرجات Silness and Löe. تم استخدام التغييرات في النسب المئوية لمقارنة التغييرات في مؤشرات اللثة والجير بين المجموعات الثلاث حيث تمت مقارنة كل تغيير سجل لكل شخص عند T1 مع ما سجل له عند T0. |
